The Adult Day Care Center located @ Waters Place in the Bronx, is seeking a temporary Social Worker to join our team! This position is 30 - 37.5 hours per week, Monday - Friday, 8:30 AM - 3:00 or 4:30PM, including alternating Saturdays 8:30 AM - 3:30 PM.
Job Responsibilities:
- Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in our Dementia Care our Enhanced Memory Impaired program.
- Coordinate and facilitate care planning and completion of all documentation
- Facilitate team and family meetings, education on older adult's rights and well-being, needs of the vulnerable elderly population. This involves extensive work with family members and friends.
