Responsibilities

The Specialty Behavioral Health Oncology Nurse role is focused on supporting members living concurrently with cancer and with high-complexity, persistent, and multifactorial behavioral health needs. This role will partner closely with our Care Team, including Social Work and Community Health Workers, to address barriers related to behavioral health - serving as the nursing expert, addressing symptom management and clinical education. This role reports to our RN Care Team Lead. 

In this role, you will conduct clinical assessments, monitor for changes in health, coordinate care, and educate members and caregivers about their diagnosis and treatment using telemedicine and telephonic platforms to support our members as they move through the oncology care continuum. You will demonstrate a strong clinical focus that supports the need for culturally competent care. Additionally, you will help improve Thyme Care’s service offerings by communicating member and provider feedback to our clinical leadership. You will also assist with other administrative projects as needed.

Responsibilities

  • Provide ongoing, longitudinal nursing support for members whose behavioral health concerns range from mild adjustment related coping support needs to more severe mental illness. 
  • Monitor member progress, provide regular updates, and establish targeted support plans with the healthcare team in case conferences.
  • Build strong, trusting relationships with providers to optimize care and prevent readmissions for our members.
  • Conducting telephonic assessments, including pain assessments and medication reconciliation.
  • Partner with non-clinical Care Team members to support the member’s social determinants of health needs, such as food resources, transportation access, and support at home.
  • Ensure members have access to medications and appointments, providing referrals and support as appropriate.
  • Be available for urgent clinical escalations and clinical consult support. 
  • Provide timely, high-quality care to members while upholding established productivity, quality, responsiveness, and performance expectations
  • Monitor assigned queue items, scheduled calls, and Slack communications to ensure timely follow-up and documentation
  • Route urgent or complex issues to the appropriate care team members or leaders
  • Adhere to Care Team policies, procedures, clinical protocols, and documentation standards to maintain quality and operational efficiency
  • Maintain familiarity with Thyme Care systems, tools, technology, and external partners to effectively perform daily responsibilities
  • Prepare charts in advance with up-to-date member information and actively participate in interdisciplinary case conferences by listening to and incorporating cross-functional perspectives, sharing nursing assessments and recommendations, and aligning care plans to support coordinated, well-informed patient care
  • Partner with non-clinical Care Team members to address social determinants of health, including food access, transportation, and other supportive resources
  • Participate in huddles and pod meetings to provide updates and support coordinated care delivery
  • Collaborate with cross functional stakeholders such as care partners, social workers, medical directors, nurse leaders and external stakeholders such as providers and payers specific to each member
  • Contribute to a team culture grounded in adaptability, resilience, professionalism, and a solutions-focused mindset
  • Approach challenges with flexibility and accountability, maintaining composure in a fast-paced and evolving environment
  • Communicate respectfully and constructively with teammates and cross-functional partners

Our salary ranges are based on paying competitively for our size and industry, and are one part of the total compensation package that also includes equity, benefits, and other opportunities at Thyme Care. Individual pay decisions are based on several factors, including qualifications, experience level, skillset, and balancing internal equity relative to other Thyme Care employees. The hourly rate for this role is $45.67. The salary range could be lower or higher than this if the role is hired at another level. This position is also bonus-eligible.
