OVERVIEW OF PRIMARY RESPONSIBILITIES:
The Registered Nurse - Community Housing & Behavioral Health Services provides comprehensive nursing services across TGCW Behavioral Health programs and Community Housing programs throughout Westchester County. The RN supports individuals with mental health, substance use, and co-occurring medical conditions in clinic, urgent care, community, residential, and scattered-site housing settings.
This position coordinates population-based healthcare activities, nursing assessment, triage, medication management, health education, disease prevention, infection control, care coordination, and wellness support. The RN collaborates with psychiatrists, therapists, medical providers, housing staff, care managers, and community partners to ensure continuity of care and regulatory compliance.
The RN serves as a clinical resource for staff, supports individuals across lifespan, facilitates access to medical and behavioral health care, and promotes recovery-oriented, trauma-informed services in both housing and behavioral health settings
PRIMARY FUNCTIONS:
Clinical Care & Assessment
Perform nursing assessments and triage across Behavioral Health Urgent Care, clinic, Sunrise, and housing programs.
Conduct health histories and nursing evaluations.
Monitor, document, and communicate changes in physical and behavioral health status.
Identify medical and psychiatric risk factors and coordinate appropriate interventions.
Complete Health Assessment Forms and social determinants of health screenings.
Assess individuals during medical emergencies, obtain vital signs, and coordinate emergency services as needed.
Medication Management
Administer medications and treatments per physician orders and nursing standards.
Monitor medication effectiveness, side effects, and adherence.
Provide medication education to individuals and families.
Coordinate medication services during transitions of care.
Support administration of long-acting injectable medications in clinics and community settings.
Ensure medication practices align with OMH, OASAS, DOH, and agency requirements.
Community Housing Services
Provide health teaching to housing participants.
Educate participants on disease management, wellness, infection prevention, and medication adherence.
Coordinate care with medical providers, specialists, and behavioral health teams.
Monitoring residents’ healthcare needs to support successful community living.
Facilitate monthly training and health education for housing staff.
Review medical records and ensure accurate electronic documentation.
Care Coordination & Interdisciplinary Collaboration
Collaborate with psychiatrists, therapists, primary care providers, housing staff, care managers, and support staff.
Participate in interdisciplinary treatment planning and case conferences.
Coordinate referrals, follow-up care, discharge planning, and outreach.
Serve as a liaison between healthcare providers, families, housing staff, emergency departments, and community partners.
Provide nursing expertise for medical and psychiatric concerns.
Safety, Compliance & Quality
Maintain compliance with agency, OMH, OASAS, DOH, HUD, and housing regulations.
Support infection control initiatives and nursing supply management.
Maintain medication and clinical storage areas in accordance with standards.
Completes required documentation in electronic health records.
Support quality improvement initiatives and regulatory readiness activities.
Education & Advocacy
Educate individuals and families about health conditions, medications, wellness, recovery, and self-care.
Advocate for access to healthcare services and community supports.
Provide trauma-informed, recovery-oriented, and culturally responsive care.
Support wellness, prevention, and population health initiatives.
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
Other additional nursing responsibilities as required by the Nursing Director.
Travel throughout Westchester County and surrounding service areas to provide nursing coverage.
Provide support to Behavioral Health Urgent Care, Clinic, ACT, PROS, OASAS, and Housing programs as operationally needed.
Perform other duties assigned by the Director of Nursing
QUALIFICATIONS AND ATTRIBUTES:
Experience with adults in Mental Health and/or closely related field
Valid CPR/BLS/AED certification
Must have reliable working vehicle, and valid clean driver’s license
Strong written and verbal communication skills
Self-Starter
High level of patience, empathy, and compassion
Excellent observation skills with high attention to detail
Strong organizational skills
Excellent problem solving and time management skills
Instructing people supported and staff in medical procedures and healthy lifestyle
Understanding of healthcare state standards and procedures
Previous clinical experience
Strong computer skills
EDUCATION AND EXPERIENCE:
Current RN license to practice in NYS
Two years of previous nursing experience with mental health, (substance abuse, chronic medical conditions, public or community healthcare)
