Sr. Supported Housing Case Manager

Location: Middletown, NY Hours: 8:30 a.m - 5:00 pm Pay: $23.50   OVERVIEW OF PRIMARY RESPONSIBILITIES:   The Supported Housing Coordinator will assist in the development, supervision, monitoring, and evaluation of the Supported Housing program, including program goals and outcomes. The Coordinator will provide supervision and support to Supported Housing team members, assist with staff training and regulatory compliance, and ensure the quality and timeliness of program documentation. The Supported Housing Coordinator will maintain a smaller caseload and provide required services and documentation to recipients. The Coordinator will work collaboratively with staff, recipients, and community partners to support the delivery of quality services and ensure that program and recipient needs are met.   PRIMARY FUNCTIONS: Supervise Supported Housing team members, including conducting performance evaluations. Provide staff training and monitor regulatory compliance. Facilitate staff meetings and generate and review reports as needed. Represent the agency at community meetings, including SPOA. Maintain a lesser caseload and complete all required documentation.  Provide Quality Assurance (QA) review of staff documentation to ensure quality and timely input into the electronic health record.  Assist with program development, monitoring, and evaluation of program goals and outcomes. Coordinate and collaborate with community resources and service providers as needed. Work a flexible schedule, including evenings and weekends as needed, to ensure the needs of all recipients are met. Participate in off-hours on-call and emergency response responsibilities.   ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT: Additional assignments may be assigned at a future date.     QUALIFICATIONS AND ATTRIBUTES: Strong supervisory, organizational, and communication skills.  Effective collaboration with staff, recipients, and community providers. Strong crisis management and problem-solving skills. Effective time management and ability to manage multiple responsibilities. Flexibility to work evenings, weekends, and participate in off-hours on-call and emergency response as needed.   EDUCATION AND EXPERIENCE: Minimum of a Bachelor’s degree required. Minimum of two (2) years in the provision of mental health services, including promoting recovery, providing crisis management, and coordinating and collaborating with the community. Supervisory experience preferred.
