OVERVIEW OF PRIMARY RESPONSIBILITES:
We are looking for a dynamic Youth Assertive Community Treatment (ACT) Licensed Mental Health Professional who is passionate about working closely with youth ages 10–21 with acute behavioral health needs. The Youth ACT Team is designed to address the significant needs of children/youth who are at risk of entering, or returning home from, high-intensity services such as inpatient or residential treatment, through the use of a multidisciplinary team.
Youth ACT services are delivered in the home or other community-based settings, with the goal of keeping youth stabilized in their homes, schools, and communities. Services are intensive, highly coordinated, and individualized, employing skilled therapeutic interventions through an integrated team approach. The Licensed Mental Health Professional is responsible for providing clinical treatment and collaborating with team members to deliver the level of care necessary to sustain progress achieved during crisis response or other out-of-home interventions.
PRIMARY FUNCTIONS:
The Youth ACT Licensed Mental Health Professional will:
Conduct assessments and provide person-centered, trauma-informed, and culturally humble treatment, including crisis intervention.
Participate in daily clinical case conferences, staff meetings, and other internal or external program-related meetings, including coordination with service providers, schools, and family members.
Ensure timely and thorough completion of documentation in electronic health records and case files.
Participate in a weekly on-call rotation, at least once per month.
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
Support team collaboration and communication to ensure quality care delivery.
Engage in professional development and training as required by the organization or program.
Participate in quality improvement initiatives and agency-wide projects as appropriate.
Flexibly provide coverage and support to team members as needed to maintain continuity of care.
QUALIFICATIONS AND ATTRIBUTES:
Ability to manage multiple work demands effectively and professionally.
Demonstrated capacity to work independently and collaboratively within a team.
Strong motivation, organizational skills, and openness to adapting for continuous improvement.
Excellent writing and documentation skills.
Proficient in using Electronic Health Records and computer systems.
Bilingual in English and Spanish preferred, but not required.
EDUCATION AND EXPERIENCE:
Must hold a current NYS license in one of the following categories: Psychologist, Clinical/Masters Social Worker, Marriage and Family Therapist, Mental Health Counselor, or Creative Arts Therapist.
Experience working with youth and families in both individual and group settings.
Preferred experience providing services in community-based environments.
Familiarity with the Ulster County System of Services is a plus.
