ACRS seeks a full-time Care Coordinator I (Vietnamese speaking) in our Aging and Adult Services department who supports highly vulnerable adults who are elderly and/or living with disabilities with very complex health and social needs to improve their health by: 1) reducing gaps in services and increasing coordination of all service providers including medical, behavioral health, long-term services and supports, and other social services, 2) providing information and education about the healthcare and social services systems and chronic disease management to help them become more confident in managing their own health and medical needs, 3) empowering them to be an active participant in the care planning process.  ***Schedule: Mon-Fri, no weekend work!   COMPETITIVE BENEFITS PACKAGE INCLUDES: Pay range is between $58,900 to $63,700 annually. Can increase based on experience. ACRS pays 100% for employee only HMO/Virtual Plus medical benefits.  Additional options are covered anywhere from 75% to 85%. Vision insurance is available at a very minimal cost. ACRS pays 100% of your life, AD&D and long-term disability insurance on or coinciding with your date of hire. Employer paid ORCA card and vanpool program. Free on site parking available at Main Office Paid Time Off: 8 hours of vacation accrued monthly from day 1, available to use after 6 months of employment. 6 hours of sick leave, accrued monthly from day 1, available to use once accrued, no waiting period. 32 hours (max) of wellness leave per year, available to use upon hire, no waiting period. Wellness leave hour amount is subject to proration based on the calendar month of hiring. 13 paid holidays annually. IT equipment provided, with telecommuting reimbursement available. 403b Retirement Savings Account eligibility. Simplified Employee Pension available after 2 years. Employee Assistance Program.   ESSENTIAL JOB FUNCTIONS: Conducts in-person visits monthly at client’s home, healthcare facility, social service agency, and/or community settings by client preference to ensure continuity of care and coordination and to reduce gaps in services. Administers assessments quarterly, develops Health Action Plans with measurable objectives, and provides health coaching to enhance clients' self-management skills and engagement with care providers. Identifies resources, manages referrals, assists with maintaining eligibility for essential services, and fosters communication between healthcare providers and networks. Supports transitional care after hospital discharges and recognizes the role of family, caregivers, and decision-makers in navigating healthcare and social services.  Ensures accurate and timely documentation and billing procedures in reporting activities and services provided.  Outreach to potential clients who are referred to the programs by explaining the benefits of the program and importance of being actively engaged in their own health and medical needs. Assists potential clients and their families in completing the required enrollment documents.  Participates in program’s quality improvement, contract compliance, and client service excellence initiatives. Ensures timely communications with program management and care coordinators.  Engages in daily case triaging and problem solving.   Attends and participates in all agency, program and team meetings, in-service trainings, and supervision.   POSITION QUALIFICATIONS: Education and Experience: Bachelor's degree (social work, behavioral or health sciences preferred) and appropriate bilingual or bicultural skills relevant to the job;  OR Bachelor's degree and appropriate bilingual or bicultural skills relevant to the job, and two (2) years paid on the job social service experience;  OR Certified Medical Assistance with AA degree and at least 4 years of paid job experience.  Cultural Humility: Strong knowledge of A&NH/PI cultures is required. Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices. Prefer fluency in an A&NH/PI language. Vietnamese preferred. Knowledge and Skills: Must complete required Health Home Care training (12 training modules and register for 1 day in person basic information training) Knowledge of the aging process; the aging network service delivery system; other service delivering systems in the community Ability to work with limited supervision and considerable self-direction.  Technical Skills: Effective ability to navigate a personal computer. Competency in use of Microsoft Office and other relevant software for word processing, calendaring, e-mail, and data entry. Efficiently research and perform various tasks via the internet.  Additional Requirements: Willingness to travel within King County (or Snohomish County) to visit clients in the community.  Must have reliable transportation. If using a personal or an agency vehicle to fulfill job duties, must have a valid driver’s license and be insurable under ACRS’s auto policy. If using a personal vehicle, must have current auto insurance including coverage for work-related business.   WORKING CONDITIONS: The position requires regular mobility within the office or workspace, may involve sitting for extended periods throughout the day and the ability to handle objects, tools, or controls. Occasional lifting or moving of up to 20 pounds may be required. May involve tasks where visual acuity is beneficial.  This position is hybrid, with work divided between remote and on-site locations. Approximately 20% of the work will be conducted on-site or in-person (home visits), while the remaining 80% will be performed remotely (telecommuting). These hybrid work expectations are subject to change at the agency’s discretion. The noise level in the on-site work environment is moderate.
