Path Assist is our tech-enabled Community Health Worker program for addressing HRSN utilizing an evidence-based, structured intervention. Our goal is simple: address individuals’ unmet HRSNs, increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend.
This community-focused Registered Nurse (RN) will travel to members' homes across Massachusetts to complete in-person health assessments for a managed care health plan's dually eligible members. The role specializes in conducting timely, accurate assessments and documenting findings, then handing off to the health plan's care management team. This is assessment and documentation work, not care plan development or ongoing care management.
Responsibilities
~1 min read- →Conduct In-Person Assessments in the Field: Routinely travel to members' homes and community settings to engage directly with members and perform required assessments (e.g., Uniform Core, Functional, and Comprehensive Assessments) evaluating physical, cognitive, and functional status while respecting cultural and linguistic needs.
- →Comprehensive Care Management Documentation: Document assessment findings accurately in the health plan’s care management platform (GuidingCare) following established workflows, and send prompt SOAP notes to the assigned Care Manager.
- →Maintain High Quality & Timely Submissions: Submit complete documentation within 48 hours of each assessment, consistently meeting health plan quality standards and resolving any audit or review findings within two business days.
- →Uphold Safety & Mandated Reporting Standards: Follow field-safety guidelines during independent home visits, escalate clinical or safety concerns appropriately, and adhere to state mandated reporter laws for abuse, neglect, or exploitation.
- →Ensure Regulatory & Compliance Alignment: Adhere strictly to HIPAA privacy rules, NCQA standards, and applicable regulatory requirements while completing all ongoing mandatory training to maintain platform access.
Requirements
~1 min read- Location & Travel: Mobile, field-based role requiring 50%+ travel to members' homes and community settings across the assigned Massachusetts territory.
- Vehicle & Licensing: Active driver’s license, reliable personal vehicle, and proof of insurance required. Employment is contingent upon a clear driving record check and maintaining continuous auto coverage required.
- Work Environment & Physical Demands: In-home visits involve exposure to varied residential environments, changing weather, and extended periods of sitting, standing, or driving.
- Schedule Flexibility: Primary hours fall during set daytime slots based on member availability, with optional evening or weekend scheduling depending on shift selection.
- Health Requirements: Annual influenza vaccination required during flu season (Oct 1–March 31) as a condition of employment, per health plan policy. New hires must submit proof within 30 days (reasonable accommodation requests evaluated per applicable law).
- Active Massachusetts RN license (or higher, e.g., NP) in good standing required.
- Associate Degree in Nursing required; BSN preferred.
- 1+ years of home health clinical experience required.
- 1+ years of assessment experience preferred (RAC-CT or similar certification a plus).
- Experience with dual-eligible, Medicaid, or Medicare Managed Care populations preferred.
- Sound clinical judgment with strong organizational and decision-making skills.
- Valid driver’s license, reliable personal vehicle, and active auto insurance required.
- Comfortable working independently in members’ homes and managing a daily field schedule.
- Proficient with electronic documentation systems via secure virtual desktop environments.
- Strong interpersonal skills with the ability to build trust with members and families.
- Cultural sensitivity and awareness when engaging diverse populations.
