Responsibilities

As an RN Admissions Nurse, you will play a critical role in helping patients and families begin their hospice journey with confidence and support. Responsibilities include:

  • Conducting pre-admission evaluations to determine hospice eligibility and appropriateness of services
  • Educating patients and families about hospice care and available services
  • Facilitating the admission process and obtaining informed consent
  • Performing comprehensive nursing assessments
  • Developing initial plans of care based on patient and family needs
  • Delivering interventions that promote comfort, dignity, and quality of life
  • Collaborating with interdisciplinary team members to ensure exceptional patient care

Requirements

  • Graduate of an accredited Registered Nursing program
  • Current RN license in the state of Ohio
  • Current CPR certification
  • Hospice experience preferred (1 year preferred)
  • Strong clinical judgment and ability to make independent decisions
  • Excellent communication and interpersonal skills
  • Ability to work compassionately and professionally with patients, families, caregivers, and healthcare professionals
  • Knowledge of quality assurance and quality improvement practices
