Responsibilities

As an RN Admissions Nurse, you will play a critical role in helping patients and families begin their hospice journey with confidence and support. Responsibilities include:

→Conducting pre-admission evaluations to determine hospice eligibility and appropriateness of services
→Educating patients and families about hospice care and available services
→Facilitating the admission process and obtaining informed consent
→Performing comprehensive nursing assessments
→Developing initial plans of care based on patient and family needs
→Delivering interventions that promote comfort, dignity, and quality of life
→Collaborating with interdisciplinary team members to ensure exceptional patient care

Requirements

Graduate of an accredited Registered Nursing program
Current RN license in the state of Ohio
Current CPR certification
Hospice experience preferred (1 year preferred)
Strong clinical judgment and ability to make independent decisions
Excellent communication and interpersonal skills
Ability to work compassionately and professionally with patients, families, caregivers, and healthcare professionals
Knowledge of quality assurance and quality improvement practices
