4-day work week (10-hour shifts)
3 days off every week
Rotating overnight, weekend, and holiday coverage shared across the team to promote balance

Responsibilities

→Provide direct patient care in accordance with State Nurse Practice Act and hospice standards
→Perform comprehensive patient assessments and implement individualized plans of care
→Deliver skilled nursing services focused on comfort, dignity, and quality of life
→Administer and monitor pain and symptom management interventions
→Educate patients and families on disease progression, care plans, and symptom control
→Document patient care accurately and in a timely manner within EMR systems
→Collaborate with the interdisciplinary team (RNs, hospice aides, social workers, therapists, dietitians)
→Evaluate and adjust care plans based on patient response and evolving needs

Requirements

Graduate of an accredited Licensed Practical Nursing (LPN/LVN) program
Current LPN/LVN license in the state of Ohio
Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
Current BLS certification
Valid driver’s license, reliable transportation, and proof of auto insurance

Experience in hospice, palliative care, or home health
Strong knowledge of end-of-life care and symptom management
Familiarity with electronic medical records (EMR) systems
