- 4-day work week (10-hour shifts)
- 3 days off every week
- Rotating overnight, weekend, and holiday coverage shared across the team to promote balance
Responsibilities
~1 min read- →Provide direct patient care in accordance with State Nurse Practice Act and hospice standards
- →Perform comprehensive patient assessments and implement individualized plans of care
- →Deliver skilled nursing services focused on comfort, dignity, and quality of life
- →Administer and monitor pain and symptom management interventions
- →Educate patients and families on disease progression, care plans, and symptom control
- →Document patient care accurately and in a timely manner within EMR systems
- →Collaborate with the interdisciplinary team (RNs, hospice aides, social workers, therapists, dietitians)
- →Evaluate and adjust care plans based on patient response and evolving needs
Requirements
~1 min read- Graduate of an accredited Licensed Practical Nursing (LPN/LVN) program
- Current LPN/LVN license in the state of Ohio
- Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
- Current BLS certification
- Valid driver’s license, reliable transportation, and proof of auto insurance
- Experience in hospice, palliative care, or home health
- Strong knowledge of end-of-life care and symptom management
- Familiarity with electronic medical records (EMR) systems
