  • 4-day work week (10-hour shifts)
  • 3 days off every week
  • Rotating overnight, weekend, and holiday coverage shared across the team to promote balance

Responsibilities

  • Provide direct patient care in accordance with State Nurse Practice Act and hospice standards
  • Perform comprehensive patient assessments and implement individualized plans of care
  • Deliver skilled nursing services focused on comfort, dignity, and quality of life
  • Administer and monitor pain and symptom management interventions
  • Educate patients and families on disease progression, care plans, and symptom control
  • Document patient care accurately and in a timely manner within EMR systems
  • Collaborate with the interdisciplinary team (RNs, hospice aides, social workers, therapists, dietitians)
  • Evaluate and adjust care plans based on patient response and evolving needs

Requirements

  • Graduate of an accredited Licensed Practical Nursing (LPN/LVN) program
  • Current LPN/LVN license in the state of Ohio
  • Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
  • Current BLS certification
  • Valid driver’s license, reliable transportation, and proof of auto insurance

  • Experience in hospice, palliative care, or home health
  • Strong knowledge of end-of-life care and symptom management
  • Familiarity with electronic medical records (EMR) systems
