Responsibilities

As an RN Admissions Nurse, you will play a vital role in helping patients and families access hospice services and begin their care journey with comfort and confidence.

Responsibilities include:

  • Conducting pre-admission evaluations to determine eligibility and appropriateness for hospice services
  • Educating patients and families about hospice care and available services
  • Facilitating the admission process and obtaining informed consent
  • Completing comprehensive nursing assessments
  • Developing initial plans of care focused on comfort, dignity, and quality of life
  • Collaborating with the interdisciplinary team to ensure exceptional patient-centered care

Requirements

  • Graduate of an accredited Registered Nursing program
  • Current RN license in the state of Virginia
  • Current CPR certification
  • Hospice experience preferred (1+ year)
  • Strong clinical judgment and ability to work independently
  • Excellent communication and interpersonal skills
  • Ability to build trusting relationships with patients, families, and healthcare professionals
  • Knowledge of quality assessment and assurance practices
