→Receive and process patient referrals by phone and in person
→Explain home health services and benefits to patients, families, and referral sources
→Assist in identifying alternative services when a patient is not eligible for care
→Coordinate and schedule initial nursing assessments and admissions
→Prepare and maintain admission packets and required documentation
→Support community outreach efforts and maintain strong referral relationships
→Provide administrative and clerical support to the clinical team
→Maintain accurate patient records and documentation in a timely manner

Must be at least 18 years of age
One (1) year of experience in home health, healthcare, or a related field preferred
Strong organizational, communication, and customer service skills
Ability to manage multiple priorities in a fast-paced environment
Ability to complete documentation accurately and on time
Valid driver’s license and proof of auto insurance required
