Advantmed is a leading healthcare solutions company focused on improving healthcare outcomes through comprehensive risk adjustment, quality improvement, and patient engagement programs. We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our full-time provider network and support members across Southern Michigan.
In this role, you will perform in-home wellness visits and risk adjustment assessments for Medicare and other eligible patient populations. Your work will contribute to accurate clinical documentation, identification of care gaps, and delivery of high-quality, value-based healthcare.
The State Traveler Provider is responsible for conducting comprehensive in-home health assessments, Annual Wellness Visits, and preventive screenings while traveling throughout Southern Michigan. This role supports quality improvement initiatives, risk adjustment documentation, and patient education to enhance overall healthcare outcomes.
Responsibilities
~1 min read- Travel to members' homes and conduct comprehensive health evaluations.
- Perform Annual Wellness Visits and health assessments for members with chronic conditions.
- Complete:
- Comprehensive medical history review
- Physical examinations
- Review of systems
- Vital signs collection
- Medication review and reconciliation
- Preventive health screenings, as applicable, including:
- Hemoglobin A1C
- Spirometry
- Kidney Health Evaluation (KED)
- Diabetic Retinal Exam (DRE)
- Fecal Immunochemical Test (FIT)
- Bone Mineral Density (BMD) Screening
- Deliver preventive health education and counseling.
- Educate patients on disease prevention, medication adherence, and chronic condition management.
- Encourage informed healthcare decisions and obtain appropriate consent.
- Conduct goals-of-care discussions when appropriate.
- Promote patient engagement and participation in healthcare programs.
- Identify clinically relevant conditions and diagnoses for risk adjustment and care management.
- Document findings accurately and communicate information for post-visit summaries.
- Assist in identifying and closing quality care gaps, including preventive screenings, laboratory testing, and follow-up care.
- Collaborate with interdisciplinary teams to improve clinical quality measures and patient outcomes.
Requirements

- Approximately 75% travel is required.
- Overnight travel may be necessary based on assignment locations and member scheduling.
- Hotel accommodations, meals, and mileage reimbursement are provided for eligible business travel.
- Travel schedules will be coordinated in advance whenever possible.
- Ability to travel frequently by automobile throughout assigned territories.
- Ability to enter and work within patient homes.
- Ability to lift and carry clinical equipment as needed.
- Ability to sit, stand, bend, and walk for extended periods during patient visits.
- Adhere to HIPAA regulations and maintain confidentiality of Protected Health Information (PHI).
- Complete accurate, compliant, and timely EMR documentation.
- Follow organizational policies, clinical protocols, and regulatory requirements.
- Maintain professional licensure and certifications.
- Perform additional duties as assigned in support of organizational objectives.
- Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Michigan.
- National board certification applicable to NP or PA specialty.
- Minimum of 3 years of direct patient care experience.
- Experience in primary care, family practice, internal medicine, geriatrics, urgent care, or related settings.
- Valid driver's license and reliable transportation.
- Ability and willingness to travel extensively throughout Southern Michigan.
