We're hiring compassionate Home Health Speech-Language Pathologists (SLPs) in Oakland, Macomb, Wayne and Washtenaw counties to provide personalized, one-on-one care that helps patients improve communication, cognition, swallowing, and overall quality of life in the comfort of their homes. - Conduct comprehensive speech-language evaluations to assess communication, cognitive, and swallowing needs; develop individualized treatment plans, provide evidence-based interventions, and monitor patient progress.
- Deliver skilled speech therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
- Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
- Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize communication, cognition, swallowing, safety, and functional independence.
- Educate patients and caregivers on communication strategies, swallowing precautions, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs) to support functional recovery.
- Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.
- Degree from an accredited Speech-Language Pathology program.
- Current Michigan Speech-Language Pathologist (SLP) license in good standing.
- Home health experience preferred (minimum one year).
- Strong clinical judgment, critical thinking, and problem-solving skills.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Reliable transportation, a valid driver's license, and current auto insurance.
