→Conduct comprehensive primary care visits in patients' homes or senior living communities
→Collaborate with a multidisciplinary team to ensure holistic and patient-centered care
→Provide preventive care, manage chronic conditions, and address acute issues in a timely manner
→Collaborate with and coordinate care amongst various community partners to ensure continuity of care
→Determine and initiate specialty referrals when appropriate
→Order and interpret lab and imaging testing
→Prescribe medications including controlled substances when appropriate
→Build meaningful relationships with patients and their families to promote trust and understanding
→Collaborate with staff at various senior living communities to ensure timely and coordinated patient care

Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred)
Active and unrestricted California medical license
Experience in Geriatric Medicine is a plus
Strong communication and interpersonal skills
Experience with Epic (EHR) is a plus
Experience with Microsoft Office is a plus
Strong time management and organizational skills
Comfortable working independently and in team based environments
Compassionate bedside manner with a commitment to providing patient centered care.
