The Community Liaison-Homeless Population builds and supports relationships that assist persons living in homelessness to access needed services and resources. Working in collaboration with partner organizations this position will help facilitate linkage and system navigation to healthcare services. This position will also help build a system of care between healthcare, housing and homeless service systems by addressing system and service gaps. Position will assist with program development, consultation, resource management, and coordination of other services and agencies to best meet the needs of the persons served.

Bachelor’s degree in a Human Services related field from an accredited college/university and three (3) years of experience working with homeless populations and individuals facing complex challenges or three (3) years of progressive experience in the field of Health Care or Human Services.
Permanent resident in Wake County or surrounding counties.
- Knowledge of the Homeless Management Information System (HMIS) and Continuum of care (CoC)
- Ability to establish and maintain favorable working relationships with individuals, community stakeholders and government employees is necessary
- Ability to build rapport and trust with homeless individuals
- Demonstrated exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts

- Demonstrated knowledge of the primary purpose and functions of community agencies
$26.27-$30.76
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity
