Responsibilities

  • Complete nutritional screenings, assessments, re-assessments, and care planning for residents
  • Develop and implement nutritional care plans and dietary interventions
  • Collaborate with Dining Services to support therapeutic diets and individualized meal plans
  • Participate in IDT meetings, care conferences, wound meetings, and family meetings
  • Monitor resident weights, hydration, intake, skin integrity, and nutritional status
  • Provide nutrition education and counseling to residents, families, and staff
  • Ensure compliance with CMS regulations and dietary standards of practice
  • Maintain timely and accurate documentation within resident medical records
  • Support quality assurance and performance improvement initiatives related to nutrition services
  • Observe resident meal services and dining environments to ensure quality and resident satisfaction
  • Assist Dining Services leadership with nutrition-related policies, procedures, and regulatory compliance

Requirements

  • Bachelor’s degree in Dietetics, Nutrition, or related field
  • Registered Dietitian (RD or RDN) credential through the Commission on Dietetic Registration (CDR)
  • Current state licensure/certification as required
  • Long-term care or geriatric nutrition experience preferred
  • Strong communication, organizational, and critical thinking skills
  • Ability to work independently and collaboratively within an interdisciplinary team
  • Senior living or healthcare experience
  • Experience with CMS regulations and nutritional compliance standards
  • Experience participating in interdisciplinary team meetings and resident care planning
