Centralized Plan Coordinator
Full-Time Position

As a Centralized Plan Coordinator, you will partner with people receiving services and their support teams to develop, implement, and update individualized goals and service plans. This role plays a key part in ensuring person-centered services are delivered consistently and effectively.
Position Summary
The Centralized Plan Coordinator oversees the planning, coordination, and documentation of services based on person-centered planning principles. This position ensures that all aspects of each individual's program plan are implemented, monitored, and documented in accordance with agency standards and regulatory requirements.

Qualifications
- Bachelor's degree in Human Services or a related field
- Minimum of one year of experience supporting individuals with developmental disabilities
- Ability to meet Qualified Intellectual Disabilities Professional (QIDP) eligibility requirements
- Valid New York State driver's license with a driving record acceptable to the agency's insurance carrier
