Our RN Diabetes Nurse Educator performs professional nursing, consultation, and educational services for persons with diabetes, and provides community health education. Develop curriculum on health and act as a health care consultant by providing information on the management of diabetes for patients of all ages across the lifespan. Additionally, as an outpatient clinic RN, this role will assess and treats patient responses to actual or potential health problems through services such as case finding, health teaching, health counseling, provisions of care supportive to or restorative of life and well-being and executing medical regimes prescribed by a licensed physician, physician's assistant or nurse practitioner. Participates in a patient-centered care team that focuses on coordinated care for individual patients, supporting patients and families to manage population health, utilizing evidence-based methodologies. A nursing regimen shall be consistent with and shall not vary from any existing medical regimen. This position reports to Outpatient Services-Director of Nursing and Clinical Services.

Roles and Responsibilities:

• Provide comprehensive diabetes education to patients, families, and staff on disease management, medications, nutrition, blood glucose monitoring, insulin administration, foot care, and prevention of complications.
• Assess, monitor, and implement individualized treatment plans for patients with diabetes, focusing on disease management, complication prevention, and improved health outcomes.
• Collaborate with physicians, care coordinators, and interdisciplinary team members to ensure coordinated, high-quality patient care and appropriate follow-up services.
• Perform patient assessments, triage, and care planning while identifying changes in patient condition and communicating recommendations to providers.
• Deliver patient-centered education related to medications, procedures, chronic disease management, preventive care, and community-based support programs.
• Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR), including assessments, treatment plans, medication reconciliation, referrals, and patient communications.
• Coordinate referrals, follow-up appointments, diagnostic testing, prior authorizations, laboratory services, and other clinical office operations to support continuity of care.
• Perform clinical procedures and diagnostic testing as authorized, including phlebotomy, EKGs, immunizations, medication administration, point-of-care testing, and specialty office procedures.
• Ensure compliance with infection control standards, medication administration policies, safety regulations, emergency preparedness protocols, and applicable legal and professional requirements.
• Participate in quality improvement initiatives, community outreach programs, continuing education activities, and professional development while promoting a positive, patient-centered care experience.

Required Skills and Experience:

• Graduate of an approved RN program with current New York State license.
• Certified Diabetes Educator (CDE) or the ability to obtain certificate within two years of employment.

Preferred Skills and Experience:

• Previous nursing experience in a medical office setting preferred. Excellent communication and writing ability.
• Basic Life Support (BLS) required within 90 days of hire date.

Physical Requirements:

• Category I. Tasks that involve exposure to blood and body fluids/tissue.

Pay Range Disclosure:

• $39.90-$47.50 per hour plus benefits
