The Sr. Case Manager provides comprehensive case management services for individuals with behavioral health and substance use needs. This position coordinates care across clinical, medical, and community systems to ensure that each client’s treatment goals and support needs are met effectively and compassionately.
The Sr. Case Manager works collaboratively within a multidisciplinary team, supporting engagement, stability, and recovery outcomes through consistent advocacy, coordination, and follow-up.
ESSENTIAL
DUTIES
& RESPONSIBILITIES
-Conduct biopsychosocial assessments and contribute to treatment plan development and updates.
-Coordinate care among clinical, medical, and recovery support teams to ensure continuity of services.
-Provide case management for clients with moderate to high needs, including crisis
coordination and discharge planning.
-Facilitate interdisciplinary case reviews and communicate progress, barriers, and next steps.
-Establish and maintain collaborative relationships with community agencies, courts, and referral sources.
-Assist clients in accessing resources related to housing, employment, benefits, and
recovery supports.
-Maintain timely and accurate documentation in the electronic health record (EHR).
-Participate in performance improvement and compliance initiatives.
-Other duties as assigned. Schedule: Tuesday through Saturday 8am-4:30pm.
REQUIREMENTS Required:
-Master’s degree in social work, psychology, counseling, or related field; or three (3) years of behavioral health experience with demonstrated case management competencies.
-Working knowledge of ASAM Criteria, Stages of Change, and recovery-oriented principles.
-Proficiency in electronic health record (EHR) documentation.
-Current CPR/First Aid certification (or ability to obtain within 30 days of hire).
Preferred:
-Two (2)+ years of experience in residential or outpatient SUD/co-occurring programs.
-Training in motivational interviewing or trauma-informed care.
-CADAC or similar certification based on state requirements.

-May involve occasional travel to community partner sites or training events.
-Regular exposure to individuals experiencing emotional distress, substance withdrawal, or mental health crisis.
PHYSICAL AND MENTAL REQUIREMENTS
-Physical: Ability to sit, stand, and walk for extended periods; occasional lifting or moving of
up to 25 pounds.
-Visual/Auditory: Adequate vision and hearing to observe and respond to client needs -Cognitive: Ability to concentrate, problem-solve, and manage competing priorities
Required:
-Bachelor’s degree in social work, psychology, counseling, or related field; or three (3) years
of behavioral health experience with demonstrated case management competencies.
-Working knowledge of ASAM Criteria, Stages of Change, and recovery-oriented principles.
-Proficiency in electronic health record (EHR) documentation.
-Current CPR/First Aid certification (or ability to obtain within 30 days of hire).
Preferred:
-Master’s degree or clinical licensure (LCSW, LPC, LMFT, LADC, etc.).
-2+ years of experience in residential or outpatient SUD/co-occurring programs.
-Training in motivational interviewing or trauma-informed care.
-CADAC or similar certification based on state requirements.
