Job Summary: The Intensive Case Manager provides intensive and comprehensive case management services in Spanish to formerly homeless residents housed in the Broadway Sansome apartments, (families and single adults) and the newly opened senior building, Philip C Chin Commons; both buildings provide supportive housing to these residents with various complex medical, mental health and substance use histories. This position requires a motivated and resourceful person, who shows sensitivity to helping address the needs of a diverse, ethnic, and formerly homeless population. This position is ideal for someone who has an interest in social work, mental health, affordable housing, and non-profit work.
Responsibilities
~2 min read- →General understanding of, and sensitivity to, issues related to homelessness, substance use, mental health, and aging amongst the very low-income population
- →Strong analytical, problem-solving, and follow-through skills
- →Ability to collaborate with other social service agencies
- →Ability to maintain confidentiality of information
- →Capacity to operate independently, take initiative, and exercise independent judgment
- →Ability to remain professional in stressful situations
- →Ability to work effectively with culturally diverse populations
- →Experience working on an interdisciplinary team
- →Bachelor’s Degree in Social Work or related field
- →Three years of experience providing intensive case management services and referrals to low-income individuals and families with mental health and/or substance use issues
- →One year of experience working with families
- →Bilingual and biliterate in English/Spanish
- →Knowledge of, and sensitivity to, the needs of low-income populations
- →Experience with crisis de-escalation and the harm-reduction model
- →Experience with support or psychoeducational groups
Preferred Qualifications
- →Master of Social Work or related field
- →Knowledge of public benefits and how to screen for benefits eligibility
- →Experience providing services in a Permanent Supportive Housing environment and working in conjunction with on-site Property Management to promote housing stabilization/retention for low-income residents
- →Familiarity with the resources available in the neighborhoods and surrounding neighborhoods of the work sites
- →Prior experience measuring program outcome
