Manager Clinical Operations, BHMG Education & Standard Practice, FT, 8A-4:30P

Individual will oversee and manage the clinical programs of the Medicine practices, to include the Chronic Care Management program, Diabetes program, Transitions of Care program, Nutritional Program and Care coach programs, as well as any clinical initiatives.  Responsible for PCMH Designation from the NCQA and any other regulatory activities for the Medicine practices.

Degrees:

• Bachelors.

 Licenses & Certifications:

• Registered Nurse.

 Additional Qualifications:

·        BSN required. 

·        Leadership experience required. 

·        Oversight of clinical programs highly preferred.

Minimum Required Experience: 5 Years
