BAYADA Home Health Care is seeking a compassionate and motivated Occupational Therapist (OT) to provide one-on-one home health visits to adult and geriatric patients throughout Monmouth, Middlesex, Somerset, and Union Counties.

As a Home Health Occupational Therapist, you'll have the opportunity to make a meaningful impact by helping patients safely regain independence in the comfort of their own homes. You'll collaborate with an experienced interdisciplinary team while enjoying the flexibility, autonomy, and support that BAYADA is known for.

As a Home Health Occupational Therapist, you will:

• →Provide occupational therapy services to adult and geriatric patients in their homes
• →Complete comprehensive evaluations of patients' functional abilities, including:
• →Activities of Daily Living (ADLs)
• →Instrumental Activities of Daily Living (IADLs)
• →Strength, endurance, coordination, balance, and mobility
• →Cognitive, psychological, sensory, and motor function
• →Develop individualized treatment plans designed to maximize independence and safety
• →Assess home environments and recommend adaptive equipment and environmental modifications
• →Implement therapeutic interventions that improve function and quality of life
• →Collaborate with nursing and the interdisciplinary care team to coordinate patient care
• →Educate patients, caregivers, and family members on therapeutic techniques, home safety, and strategies to promote independence
• →Reassess patient progress and modify treatment plans as appropriate
• →Complete accurate and timely electronic documentation using an EMR system

• Current Occupational Therapist (OT) license in the State of New Jersey
• Minimum of one year of recent Occupational Therapy experience
• Home Health experience preferred, but not required
• Graduate of an accredited Occupational Therapy program
• Strong clinical assessment and critical thinking skills
• Excellent communication, organization, and time management abilities
• Comfortable using electronic medical records (EMR)
• Ability to work independently while collaborating with an interdisciplinary team
• Reliable transportation and willingness to travel throughout Monmouth, Middlesex, Somerset, and Union Counties

• $70.00–$75.00 per point ($109,200- $117,000 annual earning potential)

• $75.00–$80.00 per point
