B. Experience Requirements:
- Substantial work experience with adolescents, preferably in a residential treatment program (minimum of three years’ experience)
- Demonstrated personal knowledge, skills, and qualifications consistent with Boys’ Haven goals and Cabinet for Families and Children regulations
- Personal capability to work in a team model, including the flexibility and willingness to fill in for other team members when needed
- Desire, ability, and initiative to obtain training to grow in job knowledge and skills

- Physical Demand and Work Environment: While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, reach with hands and arms, talk and hear. The employee is required to walk or stand.
The employee must frequently lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. The noise level in the work environment is usually moderate.

- Must know basic word processing with competency required to utilize agency computer.
- Valid driver’s license and insurability by Boys and Girls Haven Insurance carrier.
- Ability to respond to telephone and other auditory stimulation.
- At least 21 years of age and legally able to work in U.S.
- Successful completion of background screening and pre-employment drug screen.
- Successful negative TB screening.
Position Summary: In addition to responsibility to perform all the duties of a Direct Care Professional, will always function as a Pre-IL team member providing active leadership for co-workers, case management duties and assistance to the Pre-IL Program Supervisor.
Responsibilities:

- Function always as a caring, therapeutic team member who serves as a positive role model in the implementation and adherence of policies and procedures of the Pre-IL Program.
- Provides training to new employees in specific areas of their job, e.g., youth counselor responsibilities, some policies and procedures, program procedures and expectations, philosophy of treatment, etc.
- Provides ongoing modeling and training to co-workers in all areas of Pre-IL care as needed.
- Actively assist youth to reach their goals by listening, teaching and counseling by following the DCBS Developmental Milestones curriculum.
- Provide youth clear expectations and directions, exercising appropriate authority when necessary.
- Deal with problems in a calm, patient and understanding manner, seeking assistance when necessary.
- Ensure youth are made aware of Boys and Girls Haven rules and consequences for infractions, allowing youth the opportunity to explain behavior before consequences are given.
- Use specific constructive feedback to youth regarding their behavior and progress.
- Participate in treatment team and display an understanding of each youth’s problems, goals and progress.
- Conduct or participate in group and individual meetings, including leading house meetings with Program Supervisor with youth to teach specific skills regarding problem solving, living, special projects and basic life skills.
- Supervise/participate in recreational activities, assisting youth in developing skills of sportsmanship, compromise, and cooperation.
- Ensure information regarding youth and family is kept confidential.
- Be familiar with contents of daily log and schedule and supervise youth, accordingly, including exchange of information with previous and on-coming staff regarding specific problems and instructions.
- Record significant events, behaviors (positive and negative), activities and youth’s movement in/out of cottage (e.g., bed checks on evening shifts, whereabouts log, sign out sheet, home visitation request, off campus work sheets).
- File incident reports, notifying appropriate parties as necessary (e.g., physical force, AWOL procedure).
- Ensure that all forms such as community adjustments, school forms, visitor logs and sign out books are used per policy, reminding youth when needed.
- Ensure that if a youth goes on pass or leaves abruptly that staff are contacting family, friends or peers to assure the youth’s safety while they are gone.
- Provides leadership and crisis communication with staff in Supervisor’s absence.
- Completes shift report
- Case Management
- Maintain case files for clients to include all documents required. Make sure clients understand program requirements, goals and treatment plans, as well as grievance forms, clients’ rights, release of information and other relevant forms.
- Ensure clients complete necessary paperwork related to Releases of Information and create Natural Support Plans with each youth to note the clients’ progress, performance, and achievements on a case management note so this information can be used when compiling case notes, case review/audit documentation, progress reports and other critical documentation.
- Assist all residents in their application for benefits and programs as their need is determined in the client’s individual treatment plan. Coordinate with client and other resources and monitor progress toward goals established in the individual treatment plan. Continue to encourage and offer support services to client in reaching their goals. Review goals monthly and document these efforts and accomplishments. Assist clients achieve educational goals. Assist in helping clients find employment or volunteer positions and track their attendance and progress.
- Maintain all phone, written or electronic communication with youth’s FSW and report back to greater team in a written format.
- With the assistance of the Program Supervisor maintain 2-4 monthly contacts for each youth served. Two collateral and two individual and document those contacts.
- Carry up to 17 youth on caseload.
- Complete according to mandated timeframe ALPS and send copy to the Children’s Review Program.
- Complete 1294 A and 1294 B for each client monthly according to timeframe and send a copy to worker with proof in the client’s chart.
- Will be responsible to be on call every other week for a week at a time and share that responsibility with the Program Supervisor.
- Assists treatment team for Pre IL bimonthly to offer updates, report progress or barriers of youth being served and collaborate with DCBS partners and provide treatment direction for Pre IL staff.
- Invites FSW’s to bimonthly treatment teams.
- Assists the Program Supervisor with the approval and obtainment of Bed Holds and will complete BGH Tracking Forms when necessary.
- As assigned, with assistance of the Program Manager, will conduct monthly fire drills and check fire safety equipment and report any problems for immediate attention/repair.
- As assigned, explain to youth all emergency procedures and ensure procedures are followed in case of emergency.
- Ensure apartments and common area including staff office are clean, neat and in good repair (e.g., using maintenance list when needed).
- As assigned, conduct apartment and building inspections.
- Exercise proper care of equipment, such as thermostats, automobiles, phones, washers and dryers.
- Properly store and label food per health standards (routinely cleaning or supervising the cleaning of storage areas).
- As assigned, coordinate, assist and supervise meal preparation, meals and cleaning with Pre IL clients in their apartments and model for staff how to do so as well.
- Wash linens, towels and youths’ laundry as required (may include teaching and supervising youth) and supervise collection and distribution of youths’ clothing.
- Transport youth in agency vehicles, including use of key sign in and out sheets, mileage logs, and being sure to leave vehicle in good condition.
- Ensure that apartment building is secured upon leaving or if left vacant, call forwarding is utilized, and cell phone is operational.
- As assigned, serve on various committees as representatives of youth and staff.
- Medical
- Ensure through teaching, monitoring, and modeling that appropriate hygiene expectations are met.
- Working with First Shift Lead at Pre IL to ensure medical, dental and mental health appointments are made and kept (transporting if needed) and noted in client’s chart (in accordance with licensing and agency standards).
- Seek and/or follow medical instructions and/or prescriptions and record as needed.
- Ensure medications are ordered in a timely manner to ensure medication administration is compliant with physician’s orders.
- Follow emergency and routine medical procedures and ensure youth are provided with medical care and prescribed medication.
- Ensure contents of medical supplies are complete per procedure.
- Documentation
- Daily document (via Extended Reach) youth’s progress, issues, and any incidents.
- Daily check of shift notes to ensure they have been completed and follow through with staff to ensure they are done.
- Any case management contact including, phone or email communication for face to face with youth, FSW or collateral contact, be recorded in a case note in Extended Reach.
- Daily document cleanliness and/or cottage maintenance issues.
- Daily document group participation, apartment issues, incident reports, medication, and staff-to-staff communication.
Work Environment: While performing the duties of this job, the employee is exposed to a standard administrative office environment. S/he will come into contact with BGH clients, staff members, Board members, volunteers and other members of the community. The noise level in the work environment is usually moderate.
