Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant or Nurse Practitioner with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. This position blends independent practice and team-based collaboration, supported by a model that values compassion, connection, and clinical excellence.
- Continuum-Based Care (Clinical Excellence): Deliver high-quality care across home, senior living, and community settings, supported by a multidisciplinary team in palliative, psych, and neurology.
- Dementia Leadership: Build deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative.
- Rural Impact: Play a direct role in closing the access-to-care gap for seniors in Pueblo, Trinidad, Rocky Ford, and Fowler — communities where mobile, in-home care makes a measurable difference.

Responsibilities
~1 min read- →Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
- →Traveling across Pueblo, Trinidad, Rocky Ford, and Fowler to extend care access to rural and underserved communities.
- →Delivering dementia-informed, relationship-based care across the continuum of care.
- →Conducting comprehensive assessments and developing tailored care plans for older adults.
- →Managing chronic conditions and performing care with skill and empathy for patients and their families.
- →Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
- →Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
- →Maintaining consistency in documentation, follow-through, and patient communication.
What We Offer
~1 min read- Active, unencumbered NP or PA license in the state you will be practicing.
- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
- Compassionate, patient-centered approach and strong clinical judgment.
- Valid driver's license and reliable transportation, with willingness to travel to Pueblo, Trinidad, Rocky Ford, and Fowler.
