The Non-Clinical RN Case Manager supports the Vermont Army National Guard (VTARNG) by coordinating and managing medical readiness and case management services for Service Members. This position serves as a liaison among Soldiers, healthcare providers, military leadership, and medical facilities to ensure timely care coordination, documentation management, medical readiness compliance, and successful case resolution. The RN Case Manager plays a critical role in supporting operational readiness, deployability, and overall Soldier wellness.

  • Primary duty location: Camp Johnson, Colchester, Vermont.
  • Standard work schedule is Monday through Friday during normal business hours.
  • Occasional support of weekend Soldier Readiness events may be required.
  • Limited travel may be required to support readiness and mobilization activities.

Responsibilities

  • Establish and maintain case management profiles for Soldiers referred by healthcare providers.
  • Coordinate all aspects of assigned cases from identification of a medical condition through return to medical readiness or administrative separation.
  • Monitor and track medical and dental readiness requirements, treatment plans, referrals, and follow-up care.
  • Facilitate communication between Soldiers, providers, units, and medical facilities to ensure continuity of care.
  • Coordinate treatment plans and medical evaluations with military and civilian healthcare providers.
  • Support Soldier Readiness Processing (SRP), Soldier Readiness Check (SRC), and Periodic Health Assessment (PHA) events.
  • Track and manage Soldiers with Medical Readiness Classification (MRC3) and Dental Readiness Classification (DRC3) profiles.
  • Request medical evaluations, profile examinations, audiology examinations, and specialty consultations as required.
  • Educate Soldiers regarding available resources to improve medical and dental readiness.
  • Maintain accurate, complete, and timely documentation of all case management activities.
  • Document all interactions, medical information received, and case actions within required timelines.
  • Upload and maintain medical records within MEDCHART, eCase, Health Readiness Record (HRR), and other approved systems.
  • Ensure medical records are maintained in compliance with Army regulations, HIPAA requirements, and privacy standards.
  • Prepare Soldier records for Medical Evaluation Boards (MEB) and Physical Evaluation Boards (PEB).
  • Prepare monthly case status reports detailing case progress and outcomes.
  • Generate monthly reports on profile initiation, extension, and closure activities.
  • Track compliance with medical readiness requirements and communicate deficiencies to designated government personnel.
  • Participate in monthly case reviews and quality assurance activities.
  • Maintain compliance with applicable Army regulations, medical readiness policies, HIPAA, and privacy requirements.
  • Communicate verbally and in writing with Soldiers regarding documentation requirements, appointments, and case responsibilities.
  • Coordinate with unit leadership regarding scheduled medical appointments and readiness concerns.
  • Obtain appropriate releases of information and consent forms as required.
  • Serve as a liaison among military leadership, healthcare providers, and Soldiers to ensure timely case resolution.
  • Current, active, and unrestricted Registered Nurse (RN) license.
  • Minimum of two (2) years of clinical nursing or case management experience.
  • Graduate of an accredited nursing program.
  • Ability to obtain and maintain a Common Access Card (CAC) and pass a Tier 1 background investigation.
  • Strong organizational, communication, and documentation skills.
  • Working knowledge of HIPAA, privacy regulations, and healthcare documentation standards.

Nice to Have

  • Bachelor of Science in Nursing (BSN).
  • Certified Case Manager (CCM) credential.
  • Previous experience supporting military healthcare programs or medical readiness operations.
  • Experience with military health systems including MEDCHART, MEDPROS, MHS GENESIS, or similar systems.
  • Familiarity with Medical Evaluation Board (MEB) and Physical Evaluation Board (PEB) processes.
