The program Coordinator II will oversee activities related to data integrity and enhancement of the value of data. May direct the movement of data across multiple systems; oversee its validation and organization and make sure that it is available to appropriate people and systems within the organization.

Responsibilities

- Establish and maintain positive relationships with patients, visitors and other employees. Interacts professionally, courteously, and appropriately with patients, visitors, and other employees. Behave in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees.

 

- Contribute to and participate in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with corrective/improvement activities, membership on CQI teams, consistent adherence to established performance standards and: adherence to the specific rules and regulations of the B.C.H.S’s Safety & Security Policies; Risk Management: Incident and Occurrence Reporting; Infection Control Policies and Procedures; Patient and Customer Service.

 

 

- Will serve as the EHS Program Coordinator II for all children in the program, including those who are identified with a disability and/or medically fragile. Responsible for creating and maintaining educational and disability plans for each EHS child receiving EI services.

 

- As the Program Coordinator II the role will be responsible for ensuring the coordination of early intervention services for all families receiving services. This includes attending EI meetings, ensuring services are being implemented in the home, and addressing any issues that may arise.

 

- Serve as a liaison between Early Intervention agencies and families to make sure that all related services are implemented and provided to families

 

- Will be responsible for establishing and maintaining relationships with community based services, such as medical clinics, daycare facilities, preschool programs, Head start programs and other community based services to assist program participants with support services based on families’ needs in relation to disabilities and early intervention services.

 

- Will assist in the coordination and implementation of staff training with Administrative staff.

 

 

- Will be responsible for the gathering and generating data on educational goals for program reports, updating policy and procedures, completion of service plans and educational goals for home visiting staff.

 

 

- Will work with the Program Director to develop specific plans for the implementation of program activities.

 

 

- Will carry a case load of minimum of 5 families and provide home visiting services.

 

- Will assist with special projects and perform other duties as delegated by the Program Director.

Requirements

- Two (2) years database application/management experience, preferably in a large medical center or healthcare environment

- Or One (1) year relevent experience with a Master's degree (Equivalent education and work experience will be considered in lieu of the Bachelor's Degree)

 

- Bachelors degree required

 

- Masters degree preferred

 

- Database applications/management, SQL and SAS

 

- Intermediate MS Office (Word, Excel), Access, Outlook

 

- Good interpersonal and presentation skills – excellent verbal and writing skills

- Excellent attention to detail and accuracy

- Excellent conceptualization, design and analyical skills
