What We Offer
~1 min readIncumbent works with homeless families and individuals with priority given to providing permanent housing opportunities and short-term rental assistance and some case coordination. The goal is to develop and implement a service plan that addresses the program participants’ needs, obstacles and objectives for ultimately achieving permanent unsubsidized housing.
Responsibilities
~2 min read- →Implement a rapid response intake process after receiving referrals from Snohomish County Coordinated Entry with a focus on families and individuals in need of minimal short-term assistance in securing and moving into permanent housing.
- →Provide short term case management services including; assessment, intake, information and referral, and housing procurement with a focus on accessing and maintaining safe and appropriate independent housing; increasing program participants’ abilities to access services to become more self-sufficient.
- →Implement strategies for developing a progressive employment plan to move participants quickly toward financial independence.
- →Establish partnerships with Scattered Site Leasing Agents to create housing opportunities for all participants, including those with negative or no rental history.
- →Develop a system for assisting participants to remain successfully housed.
- →Provide transportation for clients to services or facilities and/or help make transportation arrangements as necessary.
- →Complete all documentation and data collection (tracking for referrals, services, HMIS and outcomes) as required for client files and grant reporting.
- →Submit financial assistance requests for client rent subsidies and other approved expenses as needed.
- →Participate in community events and/or training that will enhance service delivery and community participation and awareness of the program.
- →Other work-related duties as assigned.
- →HMIS – Incumbent is expected to develop proficiency in maintaining accurate client records in HMIS, the client tracking software application used by programs throughout the agency. Proficiency includes entry of data into the client record, the editing of information as needed, and the printing of reports within HMIS.
Requirements
~1 min read- BA in social services or related field + direct service experience in case management or related duties OR AA in social services or related field + 2 years direct service experience in case management or related duties OR 4 years direct service experience in case management or related duties.
- Reliable transportation, valid Washington Driver’s License and proof of current WA Auto Insurance.
- Proof of acceptable driving record (Driving Abstract-Full Record from Dept. of Licensing).
- Possess interviewing and assessment skills.
- Be flexible in working hours and available for work outside of an 8-5 schedule.
- Demonstrate communication skills allowing for open, trusting relationships with the participants served.
- Possess basic data recording skills, record keeping.
- Ability to work both independently and as a member of a team.
