The Resource and Community Life Case Manager is a member of the service team for Family Housing Network, a housing program of  Catholic Community Services. This position assists clients in Permanent Supportive Housing at multiple properties by connecting them to benefits and resources and facilitating group activities. The aim of this position is to improve housing stability, health outcomes, and community integration for PSH clients through individual case management and service coordination, as well as group activities and classes. This position will be an expert on government services and benefits, including the process from start to finish of applying for SSI benefits, TANF, ABD, and more. The Resource and Community Life Case Manager will work closely with and build relationships with other community agencies such as DSHS and other CCS programs and support staff. This position works closely with the PSH supervisor and Housing Services Director and provides support to PSH clients at Family Housing Network (FHN).  This position requires reliable transportation with travel throughout the county to 6 different Family Permanent Supportive Housing properties and Nativity House Apartments for single adults. 

 

 

Responsibilities

  • Provide benefit and resource case management and service coordination to individuals living in the 7 PSH buildings managed by CCS HAS and FHN according to CCS Case Management policies and contract deliverables.
  • Respond to benefit and resource referrals from primary Case Managers within 3 days and meet with client for case coordination within 1 week of referral.
  • Regularly assess the needs of households in FHN and HAS PSH projects for assistance with benefits and other service coordination
  • Case Manage families as they transition out of PSH units, including working directly with clients to support in finding housing, services, or other transitional needs.
  • Create Case Plans collaboratively with clients that include household goals, strengths, barriers, action steps to achieve goals
  • Support household as appropriate with relevant action steps including but not limited to
    • Conducting research
    • Assisting with completing forms and online portals
    • Accompanying households to medical, and government appointments, phone calls, meetings, providing the level of support and empowerment appropriate to individual household.
  • Maintain individual Case Records
  • Provide or facilitate group activities, classes, information sessions, and speakers, for clients at 6 PSH Communities across Pierce County to improve housing stability, health outcomes, and community integration for PSH clients
  • Create responsive community programming by regularly gathering client and Case Manager input on the needs of community.
  • Plan, organize, promote, and execute events and activities
  • Maintain appropriate records of community building events and activities
  • Engage with clients to find creative and in-depth solutions to reduce access barriers to group activities, classes, information sessions, and speakers. Client needs may include technology support, language support, cognitive ability support, or other barriers.
  • Provide Support for PSH Case Managers, as assigned by PSH Supervisor
  • Temporarily fill in for PSH Case Managers who are on leave
  • When other PSH Case Managers have multiple households in crisis requiring signification attention, temporarily handle routine CM tasks with other clients as directed.
  • Maintain PSH Case Files and case notes as appropriate and in coordination with primary case manager.
  • Attend meetings as necessary and represent Family Housing Network and Nativity House Apartments. Employee accomplishes this responsibility by completing the following tasks:

  • Requirements

    • Minimum 5 years’ experience in Social Services with:
      • 1-year experience in Case Management related to:
        • Services to high needs, low-income/no-income families who have experienced housing instability/homelessness, behavioral health challenges or similar while maintaining healthy boundaries.
        • Knowledge of community resources and social service systems.
      • 4-years’ experience in Human Services or Social Services performing duties aligned with the major duties and responsibilities of the role.

    or

    • Associates Degree in Social Services, Psychology, Human Services, or a related field with:
      • 1-year experience in Case Management related to:
        • Services to high needs, low-income/no-income families who have experienced housing instability/homelessness, behavioral health challenges or similar while maintaining healthy boundaries.
        • Knowledge of community resources and social service systems.

     

    • Upon contingent offer of employment, the applicant must successfully pass required background checks and provide three acceptable references.
    • Must have reliable transportation and provide valid driver's license, automobile insurance and current auto registration.
    • Must have an acceptable driving record per CCS' driving policy, (required to provide current driving abstract).
    • Proof of negative TB test upon hire or proof of test within past 12 months.
    • Bachelor’s degree in Social Services, Human Services, Psychology or related field.
    • Experience in Housing First and/or Permanent Supportive Housing models.
    • Experience accessing government benefits (SSDI, SNAP, TANF, LIHEAP)
    • Experience with trauma-informed care, harm reduction, and motivational interviewing.
    • Experience with housing services, landlord/ tenant law and other housing issues.
    • Knowledge of Pierce County social services and other community resources.
    • Bilingual or multilingual skills.
    • Familiarity with HMIS or similar data systems
