As a Floating Support Recovery Worker within Change Grow Live Nottinghamshire, you will work with clients in structured treatment that are entering, or are on a path to enter a housing crisis.

 

With this role you’ll be working as part of the floating support housing team, focussing on supporting our most vulnerable individuals who have experiences of cuckooing, unstable housing, learning disabilities and/or brain injuries.

You will hold a caseload of service users who are currently in drug and/or alcohol treatment, as well as supporting colleagues in the development of their skills.

  • Establish effective communication and referral pathways for individuals who are at risk of homelessness and in contact with other health and care agencies.

  • Provide wrap around support in emergency/temporary/supported accommodations across the area.

  • Prioritise caseload depending on crisis intervention and severity of needs.

  • Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid.

  • Work closely with the local authority, Health & Social care sector, partners and any provision linked to preventing Homelessness and improve joint working.

  • Increase and enhance partnership working with primary care, mental health services, housing and other voluntary organisations who are supporting individuals at risk of losing tenancies.

  • Provide wrap around support on budgeting, education, training and employment whilst continuing to support their engagement in structured services.

     

 

 

  • An understanding and knowledge of the harmful effects associated with drug and alcohol use in relation to health, social welfare, housing, employment and personal relationships.

  • Minimum of 12 months experience facilitating successful tenancy sustainment and working to achieve positive community connections.

  • Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs.

  • The ability to work with and manage challenging behaviour.

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
