The Licensed Masters of Social Worker (LMSW) provides comprehensive social work services to patients and their families, helping them navigate emotional, psychosocial, financial, and discharge-related needs. The LMSW collaborates with the interdisciplinary care team to assess patient needs, provide appropriate support and counseling, and connect patients and families with community resources.
Responsibilities
~1 min read- →Provide psychosocial assessments, counseling, and emotional support to patients and families.
- →Assist patients and families with discharge and transitional planning, including coordination of appropriate post-acute services and community resources.
- →Educate patients and families regarding available financial assistance programs, benefits, entitlements, and community resources.
- →Identify and address psychosocial barriers that may impact a patient’s treatment, recovery, or discharge plan.
- →Provide crisis intervention and utilize appropriate de-escalation techniques when needed.

Requirements
~1 min read- SBIRT certification required or must be obtained as required by the organization.
- NYS DOH Screen Certification required or must be obtained as required by the organization.
- Master’s Degree in Social Work (MSW) from an accredited school of social work.
- Current New York State Licensed Master Social Worker (LMSW) license.
- 1–3 years of relevant social work experience required.
- Experience in a healthcare setting preferred.
- Experience in specialty areas such as dialysis, rehabilitation, substance use/ETOH, home care, oncology, or hospice preferred.
- Experience with discharge and transitional planning.
- Knowledge of community resources, entitlement programs, and eligibility requirements.
- Strong counseling, crisis intervention, and de-escalation skills.
- Experience providing psychological and emotional support to patients and families.
- Knowledge and experience identifying, assessing, and reporting suspected abuse cases.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies, and experience and position location. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
