Join our dedicated Truancy Prevention Program as a School and Community Clinical Case Manager and help students stay on track with their education. This role provides family-centered case management services that identify and address barriers to attendance, promote academic success, and strengthen family resilience. 

You will work collaboratively with school district personnel, families, and community partners across Chester County, PA, delivering evidence-informed interventions in schools, homes, offices, and community settings. The focus is on applying and transferring interpersonal and emotion-regulation skills that foster self-sufficiency, wellbeing, and sustained school engagement—while building strong, supportive relationships between families and schools. 

 

Responsibilities

  • Skill Development & Support:Apply interpersonal and emotional regulation strategies to help students and families build the skills necessary for self-sufficiency, wellbeing, and consistent school engagement. 
  • Assessment & Planning:Conduct needs assessments and develop individualized service plans that address educational, emotional, and environmental barriers to attendance. 
  • Family Engagement:Utilize a family-centered, strengths-based approach to empower families, reduce stress, and enhance capacity for consistent school participation. 
  • Collaboration:Partner with school personnel, community agencies, and other service providers to coordinate support, monitor progress, and strengthen connections between home and school. 
  • Prevention & Intervention:Implement prevention and skill-building strategies that enhance resilience, problem-solving, and communication capacities in youth and families. 
  • Documentation & Compliance:Maintain accurate, timely service records and ensure all activities adhere to agency policies, ethical standards, and regulatory guidelines. 
  • Consultation & Training:Provide consultation to schools and participate in team meetings to support the development of effective attendance improvement and family engagement strategies.

  • Minimum 1-year related experience working with children and families in fields of mental or behavioral health, child welfare, etc...
