Reports to: Access/Crisis/Diversion Director \u007C Chief Clinical Officer oversight \u007C Pay Grade 8 (Bachelor) / Grade 10 (Master)
ROLE SUMMARY
Triages incoming behavioral health and substance use crisis calls and deploys to community settings (homes, schools, shelters, libraries, etc.) to effectively manage consumers in crisis. Responsibilities include assessment, engagement, de-escalation, stabilization, safety planning, service coordination, pre-screening, and level-of-care decisions for Medicaid and indigent consumers of Monroe County. Partners with law enforcement on ride-alongs for behavioral health crisis response and interfaces with community leaders and external providers.
CORE DUTIES
Clinical Knowledge
• Assess symptom acuity, functional abilities, and risk factors to identify severity of illness.
• Evaluate psychiatric symptomatology and risk of harm to self or others.
• Document accurate DSM-based diagnostic impressions, including ICD codes and treatment recommendations, across mental illness, ID/cognitive impairments, and substance use disorders for all ages.
• Develop an evaluation of the crisis situation and formulate a safety plan that includes alternatives to hospitalization.
• Apply knowledge of special populations: developmental disability, serious emotional disturbance in children, and co-occurring disorders.
• Demonstrate knowledge of health insurance benefits, medical necessity criteria, and current MI Mental Health Code and MI Medicaid Provider Manual standards.
• Apply Duty to Warn, Child Abuse and Neglect, and Elder Abuse reporting laws and complete mandatory reports as required.
Intake & Assessment
• Screen all incoming calls and provide crisis services for adults, children, adolescents, and families; conduct face-to-face assessments per authority standards.
• Schedule admission appointments or refer to outside agencies, including coordination with law enforcement, courts, and probation officers.
• Coordinate supports and services with the consumer's primary health care provider; send a letter to the primary care physician per agency standards upon consent.
• Assess need for psychiatric consultation authorization.
• Triage and assign consumers to the appropriate clinical department; provide level-of-care authorizations for initial and continuing care.
• Maintain complete, detailed case records consistent with MDHHS guidelines, authority procedures, and third-party payer requirements.
• Deliver individual and group community education by phone or group presentation.
• Implement interdisciplinary peer review recommendations on a retrospective and prospective basis.
Emergency Services
• Complete pre-screens and assessments to determine appropriateness of emergency psychiatric hospitalization for children and adults.
• Respond to consumer requests for emergency services via phone or face-to-face contact to stabilize crisis situations.
• Arrange alternative emergency service placements when hospitalization is not warranted.
• Transport consumers when necessary.
Quality & Performance Improvement
• Identify, operationalize, and implement performance improvement indicators with appropriate approval; actively participate in agency-wide PI activities.
• Maintain individual credentials and complete training required to stay current in all essential skill areas.
• Follow oral and written direction; complete tasks by assigned due dates.
• Maintain working knowledge of applicable software applications.
REQUIREMENTS
Education —Bachelor's Level(Pay Grade 8)
Minimum bachelor's degree with current Michigan licensure.LBSW \u007C Registered Professional Nurse
Education —Master's Level(Pay Grade 10)
Master's degree in social work, psychology, or counseling with State of Michigan licensure.LMSW \u007C LP \u007C LPC \u007C LMFTFull, limited, and temporary limited licenses accepted.
Experience
2 years with MI/co-occurring populations preferred. 1 year with SED children or adults/children with developmental disabilities preferred.
Other
Valid driver's license, current auto insurance, and driving record acceptable to the Authority's insurance carrier. Proficiency in Microsoft Outlook and Word.
