This position will cover Elderplan members who are residents of assisted living and long term care facilities in Nassau and Suffolk Counties.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantage members who reside in participating long term care facilities.

• →You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives
• →Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents
• →You will act as a clinical resource to the participating providers and facilities
• →You will act as a resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process

• Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
• One to two years clinical experience in role of nurse practitioner required
• Minimum one year in geriatric specialty
• Previous clinical RN experience in acute, subacute and/or long term care setting preferred
• Prior experience in manage care a plus
• NYS RN/NP License, Registration, DEA certificates
