Case management role for home health patients
Direct patient scheduling = you control your start and end times

Requirements

RN license in the state of CT
Some experience in Home Health required
Must be willing to cover hospice overflow
US Citizen or Green Card Holder
Comfortable with traveling between patient homes (expect 20–40 minutes of driving at times)
