Responsibilities

→Complete assignments to support resident service plans and Centers' programs in accordance with Center, Division and contract requirements.

→Document all relevant shift activity in 'log book' according to Center policy.

→Review resident log daily and document resident activities daily.

→Assist in conducting telephone and walk-in intakes for prospective residents; make referrals to community resources and services.

→Provide medications in accordance with resident's medical chart; document in resident chart.

→Monitor residents' behaviors; report incidents/violations orally and in writing to Program Manager.

→Prepare and distribute meals for residents; maintain cleanliness of kitchen and dining areas on assigned shift to include but not limited to dishwashing.

→Perform housekeeping duties in resident rooms, including but not limited to cleaning, vacuuming, wiping, sanitizing, and preparation of vacant resident rooms. Serve as a mentor to Family Support Workers. Provide initial and ongoing training as required.
