The Case Manager provides non-clinical case management support for Soldiers requiring medical readiness assistance. This position ensures accurate case tracking, documentation, and communication to support deployability and readiness objectives.
- Full-Time
- 40 Hours per Week
- Monday through Friday
- 7:30 AM – 4:30 PM Eastern Time
- On-site support required
- Travel throughout Ohio may be required for SRP and Medical Readiness Events
- Assigned support for approximately 9–15 readiness events annually depending upon brigade assignment
Responsibilities
~1 min read- →Manage assigned Soldier medical readiness cases.
- →Coordinate follow-up care and appointments.
- →Maintain records within eCase, MEDPROS, HRR, and MEDCHART.
- →Process Release of Information (ROI) requests.
- →Conduct monthly case reviews.
- →Communicate with Soldiers, Units, MRNCOs, and medical personnel.
- →Support SRP and Medical Readiness Events.
- →Track and report medical readiness metrics.
Requirements
~1 min read- Licensed Practical Nurse (LPN), Licensed Social Worker, or Bachelor's Degree in a health-related field.
- Minimum one (1) year of case management experience.
- Strong communication and organizational skills.
- Proficiency with Microsoft Office applications.
