Position Summary: The Family Advocacy & Mentorship (FAM) Program supports youth (ages 0–21) and their caregivers through an integrated, strength-based model. The program is led by a dedicated team of case managers, youth coaches, and a parent partner. FAM provides tailored services such as rapid response, case management, mentorship, caregiver education, life skills development, and access to essential resources. Our approach prioritizes family engagement, safety, stability, and overall well-being through direct support and community-based advocacy. As family needs evolve, FAM services adapt to ensure continued impact and long-term positive outcomes across Sullivan County.

The Youth Coach is a key member of the FAM Team who provides direct support, family visitation support, and mentorship to at-risk youth. They work closely with youth and families in their homes and communities to build strengths, reduce the risk of out-of-home placements, provide family coaching and promote long-term stability. Youth Coaches also foster community connections, mentor life skills, coaching family and sibling visitations, and support access to essential services that address the Social Determinants of Health (SDOH).

Responsibilities:
  • Provide one-on-one mentorship to youth in home, school, and community settings.
  • Identify youth strengths and interests to develop individualized, goal-oriented support plans.
  • Promote attachment to school, family, and community through consistent engagement and advocacy.
  • Provide information to case workers needed to connect youth and families to mental health services, substance use treatment, and vocational resources.
  • Mentor youth in developing independent living and life skills.
  • Help identify and strengthen natural support and community-based relationships.
  • Coach family and/or sibling visits according to the will of the county

Requirements

  • High School Diploma or GED required.
Experience
  • 1+ year experience in child welfare, juvenile justice, community engagement, or related experience required.
  • Driver's Licensed with the ability to maintain insurance throughout employment required.
