The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Behavioral Health Clinician II provides direct clinical services, including assessment, diagnosis, and treatment of mental health and/or substance use conditions, within an integrated, trauma-informed care model. This role involves providing behavioral health services within the Stout Street Health Center and the Stout Street Recuperative Care Center and may involve opportunities to provide behavioral health services in the community and at other CCH health clinics. This role collaborates with medical providers and interdisciplinary team members to improve access, support recovery, and enhance client well-being for individuals and families served by Colorado Coalition for the Homeless. 
 
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.  The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.  Please note that a start date will be scheduled once credentialing and privileging is complete.
  • Valid driver’s license required. This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business.

  • Conduct comprehensive psychosocial and diagnostic assessments for clients and develop individualized treatment plans. 
  • Provide individual, group, and family therapy using evidence-based and trauma-informed interventions. 
  • Deliver brief counseling, crisis intervention, and safety planning as needed. 
  • Provide medication-assisted treatment support (e.g., buprenorphine) in collaboration with prescribing providers. 
  • Facilitate referrals for specialty mental health, substance use, or community-based services to ensure continuity of care. 
  • Collaborate with primary care, psychiatry, case management, and other interdisciplinary teams to coordinate holistic services. 
  • Ensure timely & accurate recording of all clinical/non-clinical services, communications, and tasks in the EHR and other systems as required while maintaining complete up-to-date charts and any future upgrades as directed. 
  • Participate in case conferences, clinical supervision, and team meetings to support professional growth and quality care. 
  • Maintain compliance with HIPAA and all applicable federal, state, and organizational policies regarding confidentiality and client rights. 
  • Performs other job duties as assigned. 
  • Minimum of 2 years’ experience providing clinical services in behavioral health, social work, or related setting preferred. 
  • Expertise in working with Substance Use Disorders strongly preferred.
  • Experience working in integrated health, community-based, or nonprofit settings strongly preferred. 
  • Strong clinical assessment, diagnostic, and treatment planning skills. 
  • Ability to deliver trauma-informed, culturally responsive care. 
  • Effective collaboration skills within interdisciplinary teams. 
  • Strong written and verbal communication skills, including documentation in electronic health records. 
  • Ability to manage crises and de-escalate challenging situations.
