Bring your eagerness to enhance the lives of others and an ability to government programs, policies, and procedures; and we’ll provide the in-depth training and support needed to be successful in this role.\r\n\r\n\r\nYOUR ROLE\r\n\r\n\r\n\r\nThe Community Nurse Coordinator is an integral member of the\r\nCommunity Options team, supporting Connecticut residents, their families, and\r\ncommunity partners, including access agencies, healthcare providers, and other\r\nservice providers. This role focuses on helping older adults and individuals\r\nwith disabilities access the supports and services needed to remain safely in\r\ntheir homes or successfully transition back into community living.\r\nMany of these services are provided through Medicaid Home\r\nand Community-Based Services (HCBS) programs and waivers, which allow\r\nConnecticut to offer community-based alternatives to institutional care by\r\nproviding flexible supports that meet each individual’s unique needs.\r\nThis position will provide nursing support and clinical\r\noversight for the Money Follows the Person (MFP) Demonstration, a federal\r\ninitiative designed to help individuals transition from institutional settings,\r\nsuch as nursing facilities, back into the community with appropriate long-term\r\nservices and supports. The Community Nurse Coordinator will play an important\r\nrole in supporting successful transitions, promoting independence, and ensuring\r\nindividuals receive services in the most integrated setting possible.\r\nThe Community Nurse Coordinator will\r\n\r\n\r\n\r\n Provide consultation\r\n and guidance to providers, participants, and their families;\r\n Coordinate\r\n authorization and delivery of home and community-based services;�\r\n Evaluate the\r\n cost effectiveness and clinical appropriateness of medical, social, and\r\n behavioral health services; and,\r\n Ensure compliance with applicable state and\r\n federal regulations.\r\n\r\n\r\n\r\nAdditional responsibilities include conducting quality\r\nassurance reviews, monitoring services and supports, providing clinical\r\nrecommendations, and participating in fair hearings related to medical\r\nnecessity determinations.\r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\nMore details can be found in the�class specification�and the 'Examples of Duties' section below.\r\n\r\n\r\n\r\nSuccessful candidates should possess knowledge of nursing\r\ncare principles and practices, clinical assessment, care coordination, and home\r\nand community-based service delivery. PURPOSE OF JOB CLASS (NATURE OF WORK)
In the Department of Social Services, Home and Community Based Services unit this class is accountable for independently performing a full range of tasks associated with the provision of professional nursing consultation to access agencies who provide case management services to consumers and their families regarding social services program-based healthcare services.\r\n
EXAMPLES OF DUTIES
\r\n\r\n Provide consultation and case management to access agencies, health care providers, clients and family members;�\r\n Coordinates authorization of delivery of services with access agencies;�\r\n Evaluates cost and clinical appropriateness of medical, social and behavioral health aspects of care;�\r\n Conducts health care case reviews for quality and care management;�\r\n Reviews and approves or denies prior authorization requests for services;�\r\n Serves as a resource to clients and health insurance companies regarding rights and responsibilities;\r\n Serves as liaison with internal agency contacts and outside providers regarding program policies and procedures;�\r\n Completes health screen assessment and determines level of care eligibility;�\r\n Evaluates care plan to ensure compliance with cost caps and federal regulations;�\r\n Reviews, evaluates and authorizes requests for medical services and equipment for clients;�\r\n Investigates client complaints;�\r\n May conduct on site quality assessment and/or compliance review;�\r\n May represent the department in fair hearings related to medical necessity determination of nursing facility level of care;�\r\n May make recommendations for policy changes;�\r\n Performs related duties as required.\r\n\r\n\r\n
KNOWLEDGE, SKILL AND ABILITY
\r\n\r\n Considerable knowledge of principles and practices of nursing care administration for variety of specialties with varied settings;�\r\n Knowledge of\r\n \r\n relevant state and federal laws, statutes and regulations;�\r\n relevant agency policy and procedures;�\r\n managed care practices;�\r\n principles and practices pertinent to community processes;�\r\n state and federal and local health agencies and programs;�\r\n \r\n \r\n Considerable\r\n \r\n interpersonal skills;�\r\n oral and written communication skills;�\r\n \r\n \r\n Ability to\r\n \r\n problem solve;�\r\n understand and communicate health programs;�\r\n utilize computer software.\r\n \r\n \r\n\r\n\r\n
MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE
Three (3) years of experience as a Registered Nurse in a position involving home healthcare, nursing homes or other community based nursing program.\r\n
MINIMUM QUALIFICATIONS - SPECIAL EXPERIENCE
Two (2) years of the General Experience must have been in the areas of client care plans or utilization review.�\r\nNOTE: For state employees this experience is interpreted at the level of a Nurse.\r\n
MINIMUM QUALIFICATIONS - SUBSTITUTIONS ALLOWED
A Bachelor's degree in nursing may be substituted for one (1) year of the General Experience.\r\n
PREFERRED QUALIFICATIONS
\r\n\r\n Clinical experience with principles and practices of nursing\r\n care administration.\r\n \r\n Experience with Medicaid.\r\n \r\n Experience with community based long term services and supports.\r\n \r\n Experience with person centered principles.\r\n \r\n Experience or coursework with Home and Community Based Waivers.\r\n \r\n Experience collaborating with providers, agencies and organizations.\r\n\r\n\r\n
SPECIAL REQUIREMENTS
\r\n\r\n Incumbent in this class must possess and retain a license as a Registered Nurse in Connecticut OR hold a Multistate or Compact Registered Nurse license.\r\n Incumbent in this class may be required to travel.\r\n Incumbents in this class may be required to possess a valid Motor Vehicle Operator's license.\r\n\r\n\r\n
