\r\n\r\n\r\n\r\nThe State of Connecticut, Department of Social Services (DSS) is recruiting five (5) Developmental Services Case Managers in the Division of Health Services, located at 55 Farmington Avenue, Hartford, CT. Some of the candidates hired for these positions must be willing to provide case management services to caseloads in the far southwest, northwest or eastern regions of the state.\r\n\r\n\r\n\r\nPosition Highlights:\r\n\r\n\r\n Full-time, 40 hours per week\r\n First (1st) Shift\r\n Monday – Friday\r\n Hybrid\r\n 8:00 a.m. - 4:30 p.m.\r\n Hartford, CT\r\n\r\n\r\nThe Role:\r\nThis Case Management position is specifically in the Autism Waiver Services Division in the Department of Social Services, Community Options Unit.� The incumbents will be assigned a caseload in one of the following regions; Northwest, Northcentral, Eastern, Western, Central, Southwest or Southcentral and may also be required to cover statewide areas outside of their assigned region.��\r\n\r\n\r\nThe ideal candidate will have experience identifying and accessing state, federal, and/or community resources available to individuals with Autism. They will also have knowledge of state and federal benefits, including Medicaid and HCBS Autism Waiver eligibility and experience in the concepts of Person-Centered Planning, self-directed supports and self- advocacy for people with Autism. PURPOSE OF JOB CLASS (NATURE OF WORK)

In the Departments of Developmental Services and Social Services this class is accountable for independently performing a full range of tasks in providing case management services for persons with an intellectual disability and their families to ensure the delivery of appropriate medical, educational/vocational, social, residential and other services and conformance with Federal Medicaid Waiver Reimbursement Program regulations and with�federal regulations when specifically assigned.\r\n

EXAMPLES OF DUTIES

\r\n\r\n\r\n Performs activities related to individual services plans for conformity with federal and/or agency standards;\r\n Coordinates and monitors the delivery of waivered services to ensure the federal requirements for reimbursement are met and maintained and monitors completion of appropriate documentation;\r\n Develops social service evaluations and service recommendations;\r\n Oversees and supports continuity of care and waiver benefits;\r\n Monitors development and ensures maintenance of individual files including required documentation;\r\n Provides information and support to individuals and families in obtaining and maintaining social services benefits to ensure continued waiver eligibility;\r\n Provides guidance and support to individual and/or family members in locating appropriate providers in the community;\r\n Convenes, chairs and facilitates interdisciplinary/planning support team meetings to develop, review and/or modify individual service plans;\r\n Coordinates integration of day program, residential, medical and other services provided to each individual and ensures service delivery;\r\n Assists with identifying the need for evaluations, scheduling of evaluations and monitors completion;\r\n Acts as liaison and provides technical assistance to service providers and monitors compliance with departmental and waiver policies and procedures;\r\n Maintains regular contact and provides supportive guidance to assigned individuals and their families;\r\n Gives information to individuals and/or call-in’s, guardians and families regarding their legal rights, departmental policies and procedures, services provided and encourages participation in service planning process;\r\n Schedules program reviews and monitors implementation of specific program recommendations;\r\n Ensures that legal and financial documents are completed in a timely manner;\r\n Informs appropriate supervisor and regional administrative staff when services are not or cannot be provided;\r\n Assists with responses to ICF surveys, inspections and IPR/UR as required;\r\n May review individual service plans for conformity to federal regulations;\r\n May coordinate the development and assist with the implementation of Plans of Correction;\r\n Performs related duties as required.\r\n\r\n\r\n

KNOWLEDGE, SKILL AND ABILITY

\r\n\r\n\r\n Considerable knowledge of services available to individuals with an intellectual disability;\r\n Knowledge of\r\n \r\n residential programs for persons with an intellectual disability;\r\n interdisciplinary approach to program planning;\r\n intellectual disability, causes and treatment;\r\n \r\n \r\n Considerable skill in facilitating positive group process;\r\n Skills\r\n \r\n interpersonal skills;\r\n oral and written communication skills;\r\n \r\n \r\n Considerable ability to understand and translate clinical findings and recommendations into program activities and develop realistic program objectives;\r\n Ability to\r\n \r\n collect and analyze large amounts of information;\r\n utilize computer software.\r\n \r\n \r\n\r\n\r\n

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

The General Experience is defined as one of the following:\r\n\r\n\r\n\r\n A Bachelor’s degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines and two (2) years of professional experience involving responsibility for developing, implementing and evaluating individualized programs for individuals with an intellectual disability in the areas of behavior, education and rehabilitation.�\r\n\r\nOR\r\n\r\n\r\n\r\n\r\n A Master’s degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines and one (1) year of professional experience involving responsibility for developing, implementing and evaluating individualized programs for individuals with an intellectual disability in the areas of behavior, education and rehabilitation.\r\n\r\n\r\nNOTE: A degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) is a degree in the field of human services, healthcare or education including but not limited to: nursing, psychology, rehabilitation counseling, special education or sociology.\r\n

SPECIAL REQUIREMENTS

\r\n\r\n Incumbents in this class must be eligible for certification as a Qualified Intellectual Disabilities Professional as required by federal regulations.\r\n Incumbents in this class may be required to possess fluency in a foreign language or sign language for designated positions.\r\n Incumbents in this class may be required to possess and retain a valid Motor Vehicle Operator’s license.\r\n Incumbents in this class may be required to travel.\r\n\r\n\r\n
