About the Role

As a member of Daymark Health’s Care Team, you will be responsible for providing virtual care, primarily via CBT (Cognitive Behavioral Therapy) to eligible patients diagnosed with cancer. You’ll be part of a multidisciplinary care team, collaborating closely with Health Partners (Community Health Workers), Nurses, and Nurse Practitioners. This role involves conducting initial needs assessments for patients, which will guide the care we provide throughout their cancer journey, and providing expertise on the community resources available to support our patients. You will also play a key role in creating care plans to address gaps in services, such as Palliative Care, Advance Care, and End of Life Planning.

Responsibilities

  • Care for patients in individual visits to address their mental health needs and concerns. 
  • Screen for unmet care needs, including clinical or medication issues, behavioral concerns, safety risks, and psychosocial well-being. Use assessment tools and templates to support these evaluations and improve them as needed. 
  • Establish trusting relationships with patients and their caregivers, using active listening and empathy as the foundation of your interactions. 
  • Stay in close contact with caregivers to monitor side effects and any changes in the patient’s function or behavior to gain insight that helps you support providers in developing medication management strategies. 
  • Consult with licensed clinical team members, such as the patient’s PCP, and triage complex or medical issues to them as needed. You will also participate in weekly patient rounds with the interdisciplinary care team. 
  • Document all patient and caregiver interactions in our EHR system. 
  • Integrating collaborative care programming into the larger Daymark care model. 
  • Experience & Qualifications. You have a bachelor's degree and Master of Social Work (MSW) or MSW-equivalent program and have an active Licensed Clinical Social Work (LCSW) license. You’ll need to have at least two years of clinical work experience, are adept at using CBT or similar techniques, and ideally have worked with patients living with cancer. It’s critical you have a working knowledge of medical terminology. You are licensed (or willing to be licensed in Rhode Island, Massachusetts and Connecticut and beyond) to deliver amazing care to our patients.
  • Comfort with Technology. You’re comfortable using Slack, Zoom, Google Suite, and EHR systems.  
  • Strong relational skills. You have an uncanny ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.  
  • Bias to action. You don’t wait for someone to tell you what to do.  You know how to identify and prioritize your initiative’s needs and do what it takes to ensure that urgent and important needs are acted on immediately. 
  • Comfort with change and ambiguity. You may not always have direction in your work, and that’s OK, because you’re comfortable figuring out what needs to be done and a path forward.  
  • Organized & Communicative. You are incredibly organized and sweat the details. You keep your team in the loop with what you’re working on and the challenges you’re facing.  
  • Professional. You are an excellent writer, and your emails are kind, to the point, and free from errors.  

The expected compensation for this role is $75,000 to $95,000 annually, depending on location & experience.

This role is fully remote. Hours are Mon - Fri, 8:30 AM to 5 PM ET.
