Summary Statement

The incumbent is responsible for assisting with intake /referral, assessment, and case management of Mental Health court clients referred by Delaware Courts. Clients are in the community & DOC. Work includes urine monitoring, front desk coverage, referring clients to appropriate care levels, following client progress, reporting to court via status hearings, entering correctional facilities for TASC client assessments, interviews & reports, assisting clients with accessing mental health, medical, housing, education and other services to support TASC, its clients, courts, or stakeholders.\r\n

Essential Functions

Essential functions are fundamental,\r\ncore functions common to all positions in the class series and are not intended\r\nto be an exhaustive list of all job duties for any one position in the class.\r\nSince class specifications are descriptive and not restrictive, employees can\r\ncomplete job duties of a similar kind not specifically listed here.  \r\n\r\n\r\nThis class is distinguished from a\r\nlicensed Social Worker as employees do not perform the following duties:\r\ndiagnose individuals for mental, emotional, behavioral, addictive, and/or\r\ndevelopmental disorders and disabilities; perform related client assessment or\r\nevaluation; provide psychotherapeutic treatment; or develop and implement\r\nassessment-based treatment plans. An employee in this class may participate as\r\na member of a multi-disciplinary team to implement behavioral health services\r\nor treatment plans in accordance with Delaware Code, Title 24.\r\n�\r\n\r\n\r\n\r\n Interviews clients, family members or\r\n any other persons deemed necessary to obtain the most complete case history\r\n possible.\r\n Provides guidance and relevant\r\n information to the client's family to enable them to facilitate solutions and\r\n promote positive interactions among family members.\r\n Evaluates information derived from\r\n client interviews, case history, and other pertinent sources and refers to\r\n proper channels for review. \r\n Prepares narrative, statistical\r\n reports and court reports as necessary.\r\n Provides input as a member of a\r\n multi-disciplinary team in the assessment of client human service needs and\r\n treatment planning, enabling the most beneficial placement in foster care, day\r\n care, nursing home, drug/alcohol rehabilitation or other special care facility.\r\n Prepares necessary forms of\r\n application for services or benefits. Contacts appropriate social agencies and\r\n coordinates referrals.\r\n Appears at court proceedings as\r\n required testifying to the social and environmental issues affecting the\r\n client.\r\n Provides technical guidance to\r\n organizations and staff regarding findings related to client care or provides\r\n consultation to other caregiving agencies. \r\n Provides consultative services to\r\n state and private agencies; may participate in community projects.\r\n Advocates for clients and families in\r\n the protection of rights; obtains consent for treatment.\r\n Uses automated information systems and\r\n computer software to research, review, evaluate, and report on health/human\r\n service data. \r\n May lead paraprofessional,\r\n professional or administrative support staff by assigning, monitoring, and\r\n reviewing work and providing on-the-job training, guidance, and direction. \r\n Communicates and interacts effectively\r\n with people across cultures, ranges of ability, genders, ethnicities, ages, and\r\n races.�� \r\n Performs other related duties as\r\n required.\r\n \r\n\r\n

Job Requirements

JOB REQUIREMENTS for Behavioral Health\r\nCase Manager II\r\n\r\n\r\nApplicants must have education,\r\ntraining and/or experience demonstrating competence in each of the following\r\nareas:\r\n\r\n\r\n\r\n\r\n\r\n Possession of a Bachelor's degree or\r\n higher in Behavioral or Social Science or related field.\r\n \r\n Six months' experience in case management which includes assessing, planning,\r\n developing, implementing, monitoring, and evaluating options and services to\r\n meet an individual's human service needs.\r\n \r\n Six months' experience in providing clinical treatment, counseling or\r\n determining eligibility for services or benefits as part of a treatment team.\r\n \r\n Six months' experience in interpreting laws, rules, regulations, standards,\r\n policies, and procedures.\r\n \r\n Six months' experience in narrative report writing.\r\n\r\n
