Summary Statement

Within this role as a Health/Human Service Case Manager Supervisor, you'll oversee a team of 7-10 staff who help determine initial/continued ongoing eligibility for all needs-based benefits issued. This includes Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children’s Program (CHIP), Subsidized Child Care, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF). As a supervisor, in addition to hiring, training, and coaching staff, you will be responsible for ensuring staff provide services promptly, efficiently, and within federal and state program guidelines.\r\n\r\n\r\n\r\n\r\n\r\n

Essential Functions

Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. Since class specifications are descriptive and not restrictive, employees can complete job duties of a similar kind not specifically listed here.�\r\nThis class is distinguished from a licensed Social Worker as employees do not perform the following duties:� diagnose individuals for mental, emotional, behavioral, addictive, and/or developmental disorders and disabilities; perform related client assessment or evaluation; provide psychotherapeutic treatment; or develop and implement assessment-based treatment plans.� An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans per Delaware Code, Title 24.��\r\n\r\n\r\n\r\n\r\n Supervises two or more merit full-time positions.  The elements of supervision include performing the following on a regular and continuing basis:  planning, assigning, reviewing, evaluating, coaching, training, disciplining, and recommending hire, termination, and promotion.\r\n \r\n Participates in program planning, implementation, and coordination to ensure that program goals and current operations are in compliance with department, federal or state laws, rules, regulations, standards, policies, and procedures.\r\n \r\n Gathers and assimilates statistics and information to ensure program compliance.\r\n \r\n Recommends and implements corrective action as necessary.\r\n \r\n Coordinates with health and human service staff, agencies, providers, and resources to provide comprehensive services for the care and treatment of clients in a timely, efficient, and cost-effective manner.\r\n \r\n Represents the agency on task forces, committees, and with the general public.\r\n \r\n Provides information to clients, the general public, community representatives, and other agencies in order to explain programs and policies.\r\n \r\n May plan and develop programs which include writing grant proposals, completing all state clearing house documents, and implementing new grants when awarded.\r\n \r\n Analyzes budget needs, develops recommendations regarding staffing, supply, and equipment, and may monitor current budgetary expenditures.\r\n \r\n Prepares statistical and program evaluation reports.\r\n \r\n Uses automated information systems and computer software to research, review, evaluate, and report on health/human service data.�\r\n \r\n Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.\r\n \r\n Performs other related duties as required.�\r\n\r\n\r\n\r\n\r\n

Job Requirements

JOB REQUIREMENTS for�Health/Human Service Case Manager Supervisor\r\nApplicants must have education, training and/or experience demonstrating competence in each of the following areas:\r\n\r\n Three years' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs. \r\n Three years' experience�in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits. \r\n Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures. \r\n Six months' experience�in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data. \r\n Six months' experience�in narrative report writing. \r\n Coursework, training, and/or work experience in staff supervision, which may include performing as a lead worker overseeing the work of others.\r\n\r\n
