Responsibilities

→Evaluate patients’ functional abilities and develop individualized treatment plans
→Provide hands-on therapy to improve residents’ and patients’ ability to perform daily activities (ADLs)
→Document progress and adjust treatment plans as needed
→Educate patients, residents, and caregivers on exercises and strategies to improve function
→Collaborate with interdisciplinary team members, including physical therapists, speech therapists, nursing staff, and social workers
→Ensure compliance with regulatory requirements and documentation standards
→Maintain accurate records using electronic medical record (EMR) systems
