Geiss Med Hospice is seeking a motivated and relationship-driven Hospice Care Coordinator / Business Development Liaison to support referral partnerships and hospice outreach throughout the Inland Empire region. Responsibilities
~1 min readAs a Hospice Care Coordinator / Hospice Liaison, you will serve as a key connection between Geiss Med Hospice and referral partners throughout the Inland Empire healthcare market.
- →Build and maintain strong referral relationships with physicians, hospitals, skilled nursing facilities, assisted living facilities, and healthcare organizations
- →Educate healthcare professionals and community partners on hospice services, patient eligibility, and end-of-life care support
- →Conduct in-service presentations and ongoing follow-up with referral sources
- →Coordinate smooth patient transitions from hospitals, rehabilitation centers, nursing facilities, and community settings into hospice care
- →Collaborate with discharge planners, case managers, nurses, and interdisciplinary teams to support continuity of care
- →Address referral source concerns promptly while maintaining exceptional customer service
- →Represent Geiss Med Hospice at healthcare networking events and community outreach activities
- →Support hospice census growth and strategic business development initiatives
- →Provide innovative ideas and feedback to improve marketing and referral outreach strategies
Requirements
~1 min read- Minimum 2 years of healthcare business development, hospice liaison, healthcare marketing, or care coordination experience
- Experience working with physicians, hospitals, insurance providers, skilled nursing facilities, and healthcare referral sources
- Strong communication, presentation, and relationship-building skills
- Excellent organizational skills with the ability to manage multiple referral relationships
- Ability to work independently and collaboratively within an interdisciplinary healthcare team
- Valid driver’s license and reliable transportation for travel throughout the Inland Empire region
Nice to Have
~1 min read- Hospice, home health, palliative care, or post-acute healthcare experience
- Knowledge of hospice eligibility, Medicare guidelines, and transitions of care
- Existing referral relationships within the Inland Empire healthcare community
