Shift Pattern: Initial pilot phase operating Monday to Friday between 9:00am and 6:00pm. Following successful completion of the pilot period (approximately 2 weeks  the service is expected to expand to operate 7 days per week between 8:00am and 8:00pm.

About the Role

We are seeking experienced and motivated Clinical Prescribing Pharmacists to join our growing remote clinical workforce.

Working as an autonomous Independent Prescriber, you will assess, diagnose and manage a wide range of primary care presentations through digital consultations. This role is ideally suited to pharmacists with strong primary care experience who are confident managing minor illnesses, prescribing independently and delivering high-quality patient care remotely.

You will work within a well-established clinical governance framework alongside a multidisciplinary team including GPs, Advanced Clinical Practitioners, Nurses, Mental Health Practitioners, Physiotherapists and Pharmacists.

Responsibilities

  • Conduct independent patient consultations via video, telephone and asynchronous digital platforms.
  • Assess, diagnose and manage a range of acute and minor illness presentations.
  • Independently prescribe within your scope of practice in accordance with GPhC standards and clinical governance requirements.
  • Review clinical information and provide safe prescribing decisions asynchronously where appropriate.
  • Recognise limitations of practice and refer patients appropriately when presentations fall outside your clinical scope.
  • Deliver high-quality, patient-centred care while maintaining excellent patient experience standards.
  • Practise in accordance with established clinical governance and prescribing frameworks.
  • Maintain accurate, timely and legally compliant clinical records.
  • Contribute to audit activity, quality improvement initiatives and significant event reviews where required.
  • Support safe prescribing practices and antimicrobial stewardship principles.
  • Participate in peer review, supervision and ongoing professional development activities.
  • Work collaboratively within a multidisciplinary healthcare team.
  • Share pharmacy expertise and evidence-based guidance with colleagues.
  • Contribute to service improvements, pathway development and innovation projects.
  • Participate in team meetings, educational events and clinical updates.
  • Promote a culture of patient-centred safeguarding practice.
  • Recognise and respond appropriately to safeguarding concerns relating to adults, children and vulnerable individuals.
  • Escalate concerns in line with safeguarding procedures and relevant legislation.
  • Support implementation of the Mental Capacity Act, Mental Health Act and associated national guidance.

Requirements

  • MPharm degree (or equivalent).
  • Current registration with the General Pharmaceutical Council (GPhC).
  • Qualified Independent Prescriber with annotation recorded on the GPhC register.
  • Minimum 1 year's independent prescribing experience.
  • Able to prescribe autonomously from day one.
  • Completion of the CPPE Primary Care Pharmacy Education Pathway.
  • Experience conducting asynchronous consultations and prescribing.
  • Experience working within digital healthcare services.
  • Experience contributing to clinical audits, quality improvement projects or pathway development.
  • Minimum 2 years' post-qualification experience working within a primary care setting.
  • Demonstrable experience managing minor illness and undifferentiated presentations autonomously.
  • Experience prescribing independently within primary care.
  • Experience conducting remote consultations via phone and video.
  • Strong understanding of GPhC standards, prescribing governance and patient safety requirements.
  • Knowledge of clinical governance principles within digital healthcare environments.
  • Understanding of safe remote consultation and prescribing practices.
  • Excellent clinical assessment and decision-making skills.
  • Strong written and verbal communication skills.
  • Ability to document consultations professionally and accurately.
  • Proficiency using clinical systems and digital healthcare platforms.
  • Ability to work independently and manage competing priorities effectively.
  • Commitment to patient-centred care and continuous professional development.
  • Ability to build effective working relationships with colleagues and stakeholders.
  • Experience supporting multidisciplinary teams through medicines optimisation expertise.
  • Experience sharing knowledge and evidence-based practice with colleagues.
  • Familiarity with digital collaboration platforms such as Microsoft Teams or Slack.
