About the Role:

Right at Home Des Moines is looking for a compassionate and skilled PRN Occupational Therapist to join our dedicated home care team in West Des Moines, IA. In this flexible per-diem role, you'll make a meaningful difference by helping clients regain independence and improve their quality of life — right in the comfort of their own homes. Responsibilities:

Conduct comprehensive occupational therapy evaluations and assessments for home care clients
Develop and implement individualized treatment plans to improve clients' functional independence
Provide therapeutic interventions focused on ADLs, fine motor skills, and cognitive rehabilitation
Recommend and train clients and caregivers on adaptive equipment and home modifications
Collaborate with interdisciplinary care teams including nurses, physical therapists, and care coordinators
Maintain accurate and timely clinical documentation in compliance with state and federal regulations
Monitor and report client progress, adjusting care plans as needed

Requirements:

Valid Iowa Occupational Therapist license in good standing
Graduate of an accredited Occupational Therapy program (MOT or OTD preferred)
NBCOT certification required
Home health or home care experience preferred
Strong clinical assessment and documentation skills
Reliable transportation and valid driver's license for home visit travel
Excellent communication skills and a patient, empathetic demeanor
