✨ YOUR ROLE — CLINICAL LEAD IN THE HOME

You are the clinical anchor for home-based care.

• Conduct in-home patient assessments and ongoing evaluations
 • Develop, update, and oversee individualized care plans
 • Supervise and support CNAs, GNAs, HHAs, and Med Techs
 • Monitor changes in patient condition and coordinate follow-up care
 • Communicate with families, physicians, and care teams
 • Ensure safe, compliant, high-quality care delivery
 • Maintain accurate and timely clinical documentation
 • Provide guidance, education, and clinical support to caregivers

💼 WHAT WE’RE LOOKING FOR

• Active Maryland RN license (or compact license eligible in MD)
 • CPR/First Aid certification
 • Reliable transportation for home visits
 • Strong clinical assessment and communication skills
 • Experience in home care, hospice, or geriatrics preferred
