The Nurse Practitioner – Quality & Education Lead is a clinical leadership role that combines direct cardiovascular patient care with responsibility for advancing clinical quality, education, and clinician development. This individual will provide high-quality, evidence-based care in a virtual environment while leading initiatives that promote clinical excellence, consistency, and continuous improvement across the care team. The role will support clinician onboarding and ongoing education, conduct peer and chart reviews, provide coaching and performance feedback, and help develop and implement clinical quality programs and standards. Working collaboratively with cardiologists, advanced practice providers, primary care providers, and cross-functional partners, the Nurse Practitioner – Quality & Education Lead will serve as a trusted clinical resource and mentor while helping ensure exceptional, patient-centered cardiovascular care.
 
Responsibilities
  • Patient Assessment: Conduct thorough patient assessments conducted via telehealth visits, including medical history reviews, physical examinations, and diagnostic tests, to evaluate cardiovascular health and identify cardiac conditions.

  • Diagnosis and Treatment: Formulate differential diagnoses and develop treatment plans for patients with various cardiac issues. Prescribe medications and order diagnostic tests.

  • Disease Management: Manage chronic cardiovascular conditions remotely, including hypertension, congestive heart failure, arrhythmias, and coronary artery disease. Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.

  • Education and Counseling: Educate patients and their families about cardiovascular diseases, prevention strategies, and lifestyle modifications. Provide counseling on medication management, diet, exercise, and smoking cessation to promote heart health.

  • Collaboration: Collaborate closely with cardiologists, PCPs and other healthcare professionals to ensure comprehensive and coordinated care for patients. Discuss complex cases and participate in multidisciplinary care teams.

  • Research and Evidence-Based Practice: Stay current with the latest developments in cardiology research and incorporate evidence-based practices into patient care. Participate in research studies or quality improvement initiatives to enhance patient outcomes.

  • Documentation: Maintain accurate and detailed patient records, including medical histories, assessments, treatment plans, progress notes, and discharge summaries. Ensure compliance with legal and ethical standards.

  • Patient Advocacy: Serve as an advocate for patients' needs and preferences, ensuring that they receive appropriate care and support throughout their healthcare journey.

  • Continuing Education: Pursue ongoing education and professional development in the field of cardiology to stay updated on the latest advancements and maintain relevant certifications

  • Quality Assurance: Participate in quality assurance programs to monitor and improve the quality of cardiology care provided within the healthcare facility.

  • Lead and support clinical quality initiatives, including peer review and quality oversight of APPs and physicians.

  • Develop, implement, and continuously improve clinical quality programs and performance standards.

  • Design and deliver onboarding, training, and ongoing education programs for clinicians.

  • Conduct chart reviews, provide performance feedback, and promote adherence to evidence-based practices and organizational protocols.

  • Serve as a clinical resource and mentor, fostering excellence in patient care and clinician development.

  • Education: Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
  • Licensure & Certification: Current, unrestricted Nurse Practitioner/APRN license and active national board certification (e.g., AANP or ANCC). Must be eligible to obtain and maintain licensure in states required by the organization.
  • Clinical Experience: Strong clinical experience as a Nurse Practitioner, with experience managing patients with cardiovascular conditions such as hypertension, heart failure, arrhythmias, and coronary artery disease. Cardiology experience strongly preferred.
  • Clinical Expertise: Demonstrated ability to independently assess patients, develop differential diagnoses, create evidence-based treatment plans, prescribe medications, order and interpret diagnostic testing, and manage chronic cardiovascular conditions.
  • Quality & Performance Improvement: Experience with clinical quality initiatives, chart review, peer review, quality assurance, or performance improvement programs. Comfortable identifying opportunities for improvement and translating findings into actionable clinical standards and education.
  • Education & Clinician Development: Experience developing or delivering clinical education, onboarding, training, or ongoing professional development for clinicians. You are comfortable providing constructive feedback, coaching peers, and supporting clinician performance.
  • Leadership: Demonstrated ability to serve as a clinical resource and mentor to APPs and other clinicians, with a collaborative approach to promoting clinical excellence and accountability.
  • Evidence-Based Practice: Strong knowledge of evidence-based cardiovascular care and a commitment to staying current with evolving clinical guidelines, research, and best practices.
  • Virtual Care: Comfortable delivering high-quality patient care through telehealth and using technology to effectively assess, educate, and engage patients in a virtual environment.
  • Communication & Collaboration: Excellent written and verbal communication skills with the ability to work effectively with cardiologists, PCPs, APPs, clinical leadership, and cross-functional teams.
  • Patient-Centered Approach: A strong commitment to patient advocacy, education, and shared decision-making, with the ability to communicate complex cardiovascular information in an accessible and compassionate way.
