About the Role

Hopscotch Primary Care is seeking a clinically skilled, mission-driven Director of Nursing Operations to lead all nursing program functions.  This role will be the day-to-day nursing clinical leader for all Registered Nurses and Care Coordinators across the Western North Carolina region.  

This role will work closely with Care Delivery and Innovation, Regional Operations, and Clinic Leadership teams to deliver best-in-class outcomes for clinical programs including Care Management (Hopscotch Plus) and Transitions of Care Programs.   

Responsibilities

Provide Nursing Leadership  

  • In partnership with Care Delivery leadership, oversee the day-to-day nursing operations, including the Hopscotch Plus and Transitions of Care Programs 
  • Monitor and execute on performance of RN-led programs, such as Hopscotch Plus and Transitions of Care Programs. 
  • Define top priorities each quarter for the nursing and care coordinator cohorts 
  • Support clinical escalation to ensure quality of care. 
  • Collaborate cross-functionally with Product, Analytics, Operations, and Population Health to drive performance of clinical programs. 
  • Collaborate with clinic leadership to support them with LPN clinical oversight 
  • Remain up to date on RN and LPN license requirements and scope of practice in regions where Hopscotch serves patients. 
  • Review and regularly update all program playbooks, one-pagers, and guides related to clinical programs that RNs and care coordinators support  

Manage and Develop a High-Performing Team  

  • Manage RN Supervisors (TOC and CMRN), including conducting weekly 1:1 meetings, identifying leadership development opportunities, and managing performance in partnership with VP Care Delivery and Innovation and People Operations. 
  • Partner with RN Supervisors to ensure each RN and care coordinator is positioned to deliver exceptional patient care through coaching, support, and data to understand areas of strength and opportunity. 
  • Recruit, interview, and oversee successful onboarding for new RNs and Care Coordinators. 
  • Lead regular RN and Care Coordinator cohort meetings and coach RN Supervisors to lead effective meetings for their direct reports. 
  • Create and deliver ongoing training to RNs and Care Coordinators on new evidence-based-protocols, as well as new Hopscotch Primary Care programs. 

Provide Strategic Vision for Nursing Program  

  • Conduct clinical reviews to understand drivers of performance for KPIs such as Hopscotch Plus ADK and readmissions to determine programmatic opportunities. 
  • Partner with the VP Care Delivery and Innovation to set annual vision for nursing programs including KPIs, clinic and patient ratios, and opportunities to continue to optimize nursing clinical programs through technology. 
  • Lead Change Management efforts with RNs and Care Coordinators as programs, workflows and tools evolve 
  • Collaborate with the Talent Acquisition and People Operations teams on recruitment and retention strategies for RNs and Care Coordinators. 

You would be a great fit for this role if you: 

  • Hold active RN license in the State of North Carolina. 
  • Have 5+ years of experience providing direct nursing care in an outpatient setting. 
  • Have 3+ years of experience in a supervisory role with people management.  
  • Experience in Primary Care and/or Transitions of Care, specifically in a value-based care setting, preferred. 
  • Comfortable leading and owning decisions related to programs in which this role has oversight. 
  • Clear understanding of care delivery systems, methods, tools, and leadership, specifically tech-enabled “at risk”. 
  • Exposure to population health and clinical programs focused on management of value-based care population. 
  • Experience developing and executing KPIs, including performance against targets. 
  • Communicate clearly and effectively with key stakeholders both internally and externally. 
  • Thrive in a fast-paced, evolving environment and are comfortable helping build structure where needed.
